oh man. it was that one gamecube game... eternal darkness or something? it was really really good, im sure you know it. anyways when your in the main house and your insanity is way up, and the house is freaking out around you. creepy creepy stuff.
^^^This :) When the insanity meter gets to unhealthy levels the game starts blatantly manipulating the player. I once walked into a room I thought I had already cleared only to be assaulted by some monster and receive a "please reconnect controller" message at the same time. This monster tore my apart as I frantically tried fixing my controller, then the screen flashed and I was once again intact and in an empty room.

Yeah, they are creepy at first, then they're fun!
OT: I guess some stuff from Bioshock as well, such as the fellow in the freezer room in Fort Frolic. (Sorry, forgot his name.)
Ice man cometh
The Iceman Cometh, Sander baby, the Iceman fucking Cometh!

OT: I found the room in the Welcome to Rapture level of Bioshock 1 scary, at the moment when Atlas tells you about the 1-2 Punch.
Another scary moment was in asthetics (not part of story). When you walk in, there is a chair with nothing in it. Then, smoke comes down and obscures your view. When it is gone, there is a body in the chair! Then, when you pick up a tonic while looking at a wall in the same room, the smoke comes again. When it is gone, there is a tall shadow right next to yours...

Aahhh, I remember those moments. It's a shame that the game doesn't have enough of them. Bioshock 2 only has like, one of those moments in total.

Bioshock was not scary to me at all, a few startles but nothing really scary. Same for FEAR, the ghost crap was more annoying than scary to me.

Doom 3 scared me, the constant whispering, a vent cover getting blown completely off with nothing in it, and stuff constantly sneaking up behind me from rooms I've already cleared. I also started shooting every corpse in the head after I saw some stand up. This one actually triggered paranoia for me.

For me, it was Bioshock, when you face the Houndini Splicers. Creepy.

I loved the bit where you first met them, scared the living hell out of me but it was great... i remember hearing it and it dissapering round corners with dead ends... then looking at the mask and turning round...

luckily the shotgun helped me get over that fear pretty quickly :)

Area 51 for the original Xbox up until you get your alien powers, that shit had people jumping out of every which hole, and making sure you crapped your pants each time.

its not really scary but playing as the pint-sized slasher in fallout 3 gave me the chills a little bit.

The original Thief, the cathedral mission where you have to get the eye. Those damn hammer haunts just wouldnt go down and the sound they made with the rattling chains, I still jump when I play it. Or constantine's mansion that place was a mindfuck. Be interesting to see what they can do with thief 4.

Well, it was that or my first encounter with that damn time demon from Prince of Persia 2: Warrior Within. Seriously, the hell was I supposed to do? I could barely out run the damn thing and there is n "beg for your dear life" option so I was kinda miffed as well as terrified.
Ah, the Dahaka... indeed a terrifying foe.

Did you get the secret ending?

you get to kill his ass

Uh, when I first played ocarina of time, the well and the shadow temple were creepy... Shut up, I was 10! At any rate, Eternal darkness had some pretty fucked up moments. Very much so. I never played silent hill 2, but 3 was also creepy as hell.

Oh, and the vault in fallout 3 where the static causes you to hallucinate and see dead people. Hell, most of those vaults were insanely fucked up...

Oh, god, nearly the entire Medical Pavilion of BioShock 1. That damn dentist that's drilling away until you get too close, so the lights flicker and he's gone, only to reappear out of the wall when you've cleared the whole room looking for him... The shotgun you pick up, causing a swarm of splicers to attack you in the dark... ANYTHING involving Steinmann... Can you tell I just replayed that bit?

Let's also not forget the first ever encounter you have with a Houdini Splicer. Following him through Arcadia until you reach a small shrine, and turning around to see him waiting, "HELLO!"

If the Big Sister in BioShock 2 wasn't so loud, then I can think of at least five more from that game.

No, because getting all the health upgrades was infuriating without a guide, and by that stage I stopped caring. I had one left, and i'd have to start again because I missed it. Grr...

I've never been really scared.
Mildly scared: exploring pretty much anywhere in Fallout 3 at night.
Moderately scared: Dead Space. Oh man. But on the 2nd playthrough nothing scared me.
