Bloodsuckers from STALKER. Sneaking round some sewers, suddenly seeing one eating the face off a soldier. Seriously creepy, and then they spot you.

I'm not sure why but i have always been terrified of the Cacodemons (big red ball monster) from doom. The first time i encountered one i ran past where it was hiding and my blood froze when i heard a hiss, i turn around and almost scream when i see it.
I guess, ontop of how it looks, it has to do with the fact that they move silently and can fly. I actually have screamed when i'm on a raised ledge shooting at enemies from afar and then suddenly one of these bastards floats up right in front of me.

+1 for the Deathclaw. Who else discovered Old Olneye waaaay before they were leveled up enough to even think about being anywhere near it? I was on the way to Vault 92 to retrieve the Stradivarius for the old lady that lives in the wastes (Agatha?) and had no fucking idea what lay just a few hundred yards away from the entrance.

"Oooh look! A new town to loot!" thought I.......

I refused to go back until the Brotherhood of Steel made me. I took Fawkes, a fuckton of heavy artillery and spare sets of armour. I became addicted to umpteen different chems in an effort to boost my combat abilities and I was still bricking it the whole sodding time I was there.

Took this topic long enough.

Red forest bloodsuckers in Clear Sky are the worst. You can hear them rustling the trees every time you go though. They don't attack you every time, but when you set foot in the forest, you better grab that shotgun.

Or the time when you sneak though a lair full of sleeping bloodsuckers.

These mean old ghosts always put me on edge. Having Aaron hunt you was a terrifying experience.

I must be going crazy because stuff that needs to make me wanna puke is completely normal to me (and all of you too). Scary dudes:

Laugh it up, if you will, but it was one of the first PC games I owned. I'd gone through the usual menagerie of Spidey villains, Venom (who deserves honorable mention for making me panic as he fights you while slowly drowning M.J,) Mysterio, Rhino, Scorpion, what have you. Nothing terribly out of place.

During the end part of the game, you fought Doc Ock, which was neat, and Carnage, which was fairly creepy.

Then the game gave Doc Ock Carnage's symbiote.
The one, and only, Monster Ock appearance.
Indestructible, smashes through anything, and chases you through an exploding base for 6 levels. Oh, yeah, that scream? He'll make it every time he catches you

And that's the scariest monster I ever saw in video games.

Any monster that smells like the rear end of a thousand unwashed hobos & threatens to throw fecal matter at you is one to be avoided.

does something bad happen if you let it keep wriggling around, or does it just not stop until you shoot it?

Due to the sheer hysterics caused by the first encounter, I'm going to call out the Hydra from the original Half-Life.
Edging slowly around the sides of the rocket test chamber, and absoluetely bricking yourself if you slip a step too far and suddenly all three arms swing round to face you. Then it's a standoff, they haven't killed you yet, but yo're not through the door. It's right there but it's boarded up, if you run for it now you can smash the planks and be through before they catch you, at least you think you can...

Agreed with the OP, Deathclaws.
First time I went on Fallout 3, I was playing on a friends game. I just happened to come across two of these things. A proper Oh Shit moment.

this one is interesting, i wouldnt have thought of it.
I remember one of the boss type guys in oblivion cast a spell that made a second of me that I had to fight, it was intesne.

also, in dragon age origns (minor spoiler) when you play as a wood elf (i forget the actual name atm) and you see your dead zombie looking friend again.

EDIT: and just about everything in Vampire: The Masquerade, Bloodlines
