The first time I saw the Lambent Brumak at the end of Gears 2, I was moderatly spooked... that is until I realized it had the resilience of a gnat with cancer.

But, some of the scarier ones:

El Gigante and "It" from Resident Evil 4
The Rancor from KOTOR
The first encounter with a thresher maw in Mass Effect
The Leviathan in Gears of War 2
The Bull Rancor from The Force Unleashed
Those almost invincible bugs in Resident Evil 5 (can't remember their name...more annoying than scary, but the cutscene when they kill you still gives me the jibblies)

The puppets from Thief 3 *shudder*

they twitch, lights flicker when they are near and when you stab them... they GET BACK UP WHEN YOU TURN AROUND!!!

I still have nightmares about em

whatever those things in eternal darkness were that hid in people's corpses while they were still able to function. eeesh. And then they'd pop out and try to crawl into yours.

Curiously it just lost a lot of its scare when Rebirth (better graphics patch) came out. I still remember it fondly, though. Along with FEAR it's one of only two games to ever make me afraid to go for a toilet break at 2am.

No, that's not promotional art (whoever took the pic added the "fatal frame II crap).
That scene is part of the game.
Yes, she did all that.
Yes, she's laughing her ass off (her laughter is the stuff of nightmares, too).
And she's doing it because her twin sister refused to kill her.

Isn't she lovely? =D

Everybody is talking about Resident Evil and Silent Hill. What about things in games that are scary and annoying? Remember those hands in the original Legend of Zelda that, once touched, would take you to the beginning of the level? I think they brought them back in later games of the series... But I remember when I saw them in a room, I would get a little scared.

I couldn't really choose at first, but I knew after only eyeing the game for a split second.

When I was younger, this was the scariest shit I had ever seen.

Barnacles from Half-Life 1 and 2. Even after death they are highly unnerving, since a glance does not always let you see if it's harmless or not.

for me its the pinatas from viva pinata. They scare the crap out of me; as soon as I played the game, I pissed my pants.

If you're after a similar experience, or at least as close as you can get in that genre; You should play Demon's Souls.
It's the only game that I felt the same creepy feeling from as I felt when I first played Diablo.

It's a hard feat to accomplish.

This. To this very day, I can not play any episode of Duke Nukem 3d besides the first one.

I used to play the other episodes with the complete map view on if I thought that there might be a slimer around. One time, during the map Going Postal (which have the most of those fucking things on any map aside from The Queen), I thought I had cleared the whole thing and turned the map view off. THEN RIGHT AT THAT MOMENT arrgh. You know the pond in Inhiding, the first map of episode 3? I used to empty my RPG twice (Dnstuff ftw) before going in.

I refused to go back until the Brotherhood of Steel made me. I took Fawkes, a fuckton of heavy artillery and spare sets of armour. I became addicted to umpteen different chems in an effort to boost my combat abilities and I was still bricking it the whole sodding time I was there.
I hated those things as well. Then I found out that the Metal Blaster (souped up Laser Rifle from the Pitt) actually works really well against them, provided you get the opportunity to sneak crit them, or have lots of AP. :D

My personal choice definitely has to be the Regenerators from Resi 4. The Infrared Scope is probably one of the ultimate godsends of my gaming life.
