Fast Zombies from the Half-Life 2 saga.

Nothing scarier than the reanimated, skinless body faster than a car rush at you screaming the creepy wolf-like scream.

And all enemies from the Penumbra series.

When it comes to the first monster that scared me in a videogame, that would be the eels in mario 64. They scared me so much that I skipped the water stages as much as I could.

Now a days I tend to stay away from the horror genre of games, though that term is starting to confuse me, as I see more and more movies that are labled horror and aren't scary in the least, like Frailty. So when it comes to my current scariest monster, I would have to go with the two legged colosous's in SotC, just because of the shear crap your pants huge they are when they're right next to you

the spider things from doom 3....when you see a swarm of those creepy little buggers coming towards you from the vents and pipes and what not, it's pretty terrifying...

Every-Fucking-thing in Ravenholm that moved. I was jumping out of my skin and pressing the trigger key every time something twitched.

That, and LAG. Nothing's scarier than facing down a Tank in L4D 2 and BAM! Spontaneous Laggage, Teleport to the next county, DEATH.

The zombies from the Shalebridge Cradle in Thief Deadly Shadows. My friend always went on about this level being the scariest he'd ever played, so I bought the trilogy. It didnt have the same effect on me, since he told me that the first half of the level has no one in it, so it wasn't as creepy since I already knew.

However, they were still terrifying. The way they make no sound themselves, but instead make all the lights flicker where they are, and the way they spasm and move around. Also, knowing the story behind the Cradle and who the zombie were also added to the complete and utter terror I felt wandering around.

I genuinely screamed when, while I was happily picklocking a door, I turn around and see a zombie charging towards me in complete silence. I spammed flashbombs and just ran.

Some things are scarier when they don't make a sound.

I hated those things as well. Then I found out that the Metal Blaster (souped up Laser Rifle from the Pitt) actually works really well against them, provided you get the opportunity to sneak crit them, or have lots of AP. :D
Gatling Laser goes through them like a knife through butter. It's just the sheer number of 'em at Old Olney that makes it crazy. I have yet to venture into the Deathclaw Sanctuary.

Maybe not needed after it's been out so long, but gameplay spoilers below

I also discovered that while the Deathclaw control gadget that the BoS give you is pretty useless at Old Olney, if you have decent Science skill and can hack higher level terminals, it works a treat at Adams AFB if you unleash the "tame" Deathclaws onto the base

Something about the fish monster that you kill with a flamethrower in Call of Cthulhu freaked me out. Those fish monsters are so unnerving...
And regenerators. The first time I encountered one, I shot it's limbs off, then it JUMPED IN THE AIR AND LATCHED ON TO MY NECK. A TORSO ATTACKED ME. Man it really freaked me out. I blasted that fucker till my shotgun ran out of ammo.

This. I'm pretty sure I blocked out the entirety of the "Bottom of the Well" level when I played it as a kid, 'cause I recently went back and played it. I pretty much went "What the fuck is this? I remember there being a level under the well, but was it really this shit? There's blood and gore and crazy arm monster things everywhere!"

I'm surprised I wasn't traumatized. Or maybe I was....
It really was a creepy level, but the Shadow Temple was even worse: the way Navi would stop you every ten seconds to tell you about the "bloody past" of Hyrule. I'm actually really surprised I played such a freaky game as a kid!

In fact, I'm really surprised that the ESRB rated it E, when Twilight Princess was nowhere near as traumatizing but managed a T rating! I wonder what rating a remake of the game would garner in this age of overprotective parents and a stricter video game ranking system.

The Great Mighty Poo from Conker's Bad Fur Day.

Any monster that smells like the rear end of a thousand unwashed hobos & threatens to throw fecal matter at you is one to be avoided.
Come on we all know he lyrics! Really he's quite the opera singer.

Oh if you love the gatling laser the more powerful variant of it resides in, guess what? The Deathclaw Sanctuary!

By which I mean, Demogorgon.

Edit:
For sheer scare value, there was a (relatively rubbish) FPS called Ominous Horizons some years back.
It had a level set in Egypt. In this level, inside a crypt, were a number of statues lining the walls. When you looked at them, they were still. If you turned around, then back, they were substantially closer. No sound, no action... just the most claustrophobic nightmare imaginable.

You had to get through several rooms and corridors of them; I could not do it.

The Pinky Demons from Doom 3 always scared the hell out of me! It wasn't so much the monster itself that scared me, but rather the creepy shit that always accompanied their arrival.

EDIT:

Oh, and the BLOODSUCKERS FROM S.T.A.L.K.E.R.!!!!!!!!!!

These things still scare the hell out of me. I don't like the whole 'chameleon' thing they can do. They come out of nowhere and eat your face!
