Falling. Even when I play a games like Spiderman 2 and Prototype where literally leaping off the Empire State Building will not kill me, I always get this queasy feeling in the pit of my stomach. And then there's Max Payne's (the first one) nightmare levels. Knowing you're going to hear that bloodcurdling backwards nightmare scream every time you fall off the little blood trail (and yes, you WILL fall off) is absolutely terrifying.

Although I know that it has already been mentioned, getting eaten by a giant fish.

More specifically, being eaten by a ravenous Lurker shark in Jak and Daxter: the Precursor legacy, as opposed to the same way you die in the other two games after it. By that I mean in Jak II you were just blown out of the water by the Barons mini sub, and in Jak3 you were pulled down by the kraken. But the Lurker shark was the worst.

Nothing got me out of the water faster than the Lurker sharks attack music.

I wouldn't go so far as to say scariest, but one of the ones that most got to me was at the start of CoD4 when that character you are seeing through the eyes of gets lead through the streets and eventually shot. And you're going "Something is gonna stop this, I'm going to be given a way to escape. This is where the game starts and I make my daring escape" and no..you look down the barrel at a man you don't know, doing things for reasons you don't know, and oblivion greets you.

I'm not sure why, but this was very true for me in Oblivion. More than anything else, that Thieves Guild mission with the wizard's tower. Swimming down that tunnel scared the hell out of me and I'm not sure why.

I was going to say "Anything from Deadspace," but this sounds worse.

Completely forgot about that scene in reference to character death...not morose in the least bit.

It isn't a specific means of death, but I always get scared shitless when I'm fighting for my life in any given game, and I'm so close to the end and I keep pushing on, and then finally something terrible happens or I screw up and that game over screen rears its ugly head.

The scarecrow sections in arkham asylum comes to mind.I just feel so helpless when the game turned from an ass kicking simulator to a fly swatting simulator were you play as the fly.

while not actually done, what this person from tsukihime, ciel, says at the end of her true ending is quite....creepy, and would be a pretty horrible way to die.

Oh god.. DP's stealth sections. They always creeped me out. I'd just be hiding under a table, when suddenly my heart-rate hits max and York lets out a stupid, "ARGHGHSTRHGH I'M FREAKING OUT MAN!!!!", leading me to the game over screen. And.. that music. Even creepier.
