This sounds silly, the one time in a game my heart literally jumped....was a Nancy Drew game. They seem harmless but my friend and I were playing. We made Nancy reaches over this balcony toward this clue. She says "I can't reach" so we click again just to see what she'll say. Suddenly the camera goes spinning and you hear her blood curdling scream as she falls and snaps her neck.

I got really scared, because I am afraid of sea life for some reason.
I hate that thing, because of that I'm afraid of any source of water big enough for me to fit in

Can't stand the ocean now, thanks evil shark.

I had gone all the way through tomb of the giants only to find that I needed some bowl thing to open the last passage to the boss. (Giant glowing wall blocking my way.)
The problem was that I had used a bonfire somewhere in there and never got the lantern so ... pitch black.
Then I get the phantom has invaded message so ... crap.
I killed him only to get taken out by one of the four-legged skeleton things. >_<

The giant fish that eats you if you go in the water for too long in "Jak And Daxter." That thing caused the development of my phobia of open water that I have today.

Particularly the eye poke machine, and the small head-with-tentacles necromorph that rips Issac's head off inserts itself and starts walking around as a zombie. That creeped me out to no end.
I first thought of Dead Space when I read the thread title. I actually think some of the death scenes are interesting, in a creepy but creative sort of way. I actually went and looked up some of the ones you all mentioned after I beat the game to see what happened.

I remember what would kill me and what scared me were either the Twitches that would go up and just tare you apart, or if I went to close to a Guardian to where a tentacle shoots out and tares Issac?s head off. I always thought those scenes were especially creepy because Issac does not die immediately. With the Guardian he is still feeling around for his head. Also, while I never died from this, I would panic once I started to hear Issac breathing heavily in a decompressed area or in space. That sound is telling you your air supply is running low, and if you do not hurry you are going to suffocate. What is worse, or better for game reasons, is that all you really hear in these areas is Issac?s breathing so you cannot help but listen and hear it get more and more labored.

Holy crap...yeah. Totally had nightmares after downloading this game and staying up playing it until 4 in the morning. Let one of my friends have a go at the game. She definitely agrees that death by SCP-173 is terrifying. Would have initially gone with eye-poke machine in Dead Space 2, but now definitely have to go with being murdered by ^^^^

OT: I hate it in STALKER when you're in a lab/underground, and then you hear the controller's psy attack wreck your brain (and ears) and then you're suddenly looking at it. And then you try and run, and it happens again. One more time, and then you're dead, and the beast continues standing in the shadows.
The event above (and the other events of common death in STALKER) kinda reminded me of how mortal and fragile humans are, and how we struggle against beasts and others that are stronger than us--our skills and weapons our only advantage.

Jumping off the Agency tower in Crackdown always made me uncomfortable.It made my stomach feel like I was falling so I quit jumping off of buildings.

i don't know if it qualifies, but as a kid i couldn't play the original prince of persia alone (my dad use to play with me from time to time) because i was scared to death by those skeletons and traps.

it wasn't before a couple of years that i finally conquered my fear - to the point that's now the game i'm (this probably sounds sad) the best at.

EDIT: Okay, okay, so it's not from a game but it's in the same web series.
That thing looks like it is trying to do the bewbs and butt pose that all female superheroes do xD Those games look awesome though. especially the staircase one. Horror game makers should take note.

I really don't like any sort of hostile water based creature in a game. My friend died to a big whale thing in wow and it totally freaked me out.

The shark in Arkham City was a bit wtf too.

Well, I haven't been killed by it yet, but the Barnacles in Half-Life are pretty damn scary. I just suddenly start going upwards and then I look up and... OH GOD OH GOD OH GOD

And I can't think of any other game that's managed that.
When I saw the thread title, I, too, immediately thought "Mirror's Edge". My favourite game ever.

That said, also high on my list of scary deaths is being walloped by a Tank on expert mode: the noise and the buildup; at times its genuinely bloody terrifying.

In horror games, the scariest way of death, by far, is from something you can't see/don't know about. One of the reasons why I hated Amnesia (ok, I really liked it, but hated it as in, "WHY THE FUCK DO YOU DO THIS TO ME, GAME?! WHYYY!? YOU ARE TEARING ME APART, FRICTIONAL!") is that you see the monsters for shit, which is good. Most of the time, you're in a closet, crying and sucking your thumb, waiting for the creepy music to stop, and you won't even see the monster. You can hear its gargled moans and grotesque pulsing, but not see it. It kind of makes you think of how the monster looks, and if anyone can scare you, it's yourself.
