All of the death scenes in Dead Space 2 that give you a nice close-up of Isaac's face, like the tripod in the church, the eye poke machine, or getting grabbed by Marker-Nicole during the final boss fight. They just disturb me to no end, and I usually pause and reload the checkpoint if I mess up during those bits rather than continue to watch. It's strange, because all of the other death scenes, where Isaac's helmet stays on, are usually pretty entertaining to me. It's just being forced to watch his terrified face (or not-so-terrified in the case of the final boss fight, which actually just makes it worse) while it happens that gets to me.
I used to be a space engineer, but then I took a javelin to the jaw.

Crysis, when I first tried to swim off the island and got a surprise visit from Mr.Shark.

I had to stop playing for an hour to calm myself.

EDIT:

It has to be that one part in Mirror's Edge where you are running through the train tracks. There's something about seeing something that big barrelling towards you that fast that scares the hell out of me. Falling off buildings didn't really have the same effect on me.

Also there's nothing quite like being humped to death by a redead.

Wow... There's some interesting and varied responses. A few people do seem to hit on a theme of death being drawn-out but inevitable. (my own example of Mirror's edge... You know you're done for as soon as you realise you're not going to reach anything you can grab. But it still takes seconds before you hit the ground.)

But it's interesting to see other things that disturb people as well.

Wait... Your Captha said "come along, pond"?

That almost sounds like it was ripped from a doctor who script...

Playing the remake of Resident Evil 1 on the Gamecube, late at night, in the dark and not knowing about the existence of Crimson Heads! I shat myself when a Zombie suddenly jumped to its feet and then ran at me full speed killing me in one swipe because my health was stupidly low. I screamed at the the television and had to stop playing for a moment as I was genuinely in shock.

I always found it really scary to have my Pokemon poisoned somewhere really far away from a Pokemon Center, then seeing it die with the Center in sight. When you play the Nuzlocke Challenge, shit gets real.

As for me, the player, dying, it'd be Amnesia. Turn around to see this disfigured thing standing behind you and die with a single stroke of his claws.

Dying from monsters in the water, that insta kill you, and you know they're in there and can't be killed.

Metro 2033: Broken Gasmask Syndrome.

I was up on the surface, micbreathing into my fogged-up mask, when I was ambushed by some mutant rats. I did manage to shoot them, but they cracked up the visir to my gasmask, a long, long haul away from a replacement one.

It was... Rather awful, staggering around aimlessly in desperation in the snow with my vision washing out and my lungs slowly shriveling up in my chest. When I did finally bite it, it felt like a liberation...

I was ambushed by an albino radscorpion in Fallout 3 once, too. I didn't even hear it, being inside a pitch-black metro tunnel. Suddenly, I turn around and I've got those beedy, black eyes staring at me. "Yo! I like violence too, check it out!" it seemed to say before I got a stinger in my face. :<
