Well, I can think of three right off the bat. One just happened about an hour ago.
1. I was half-way through the suicide mission in ME2. I apparently got too close to my biotic because she teleported. Half way across the map. I was then promptly omnomed by seekers. Apparently this made my game really unhappy because it caused another. When I restarted, my biotic died, like literally died half through the map and everything disappeared. I ran across the rest of the stage and my screen went black. Then it cut to shepard convulsing on the floor with his legs and arms stretched out into the blackness.
2. I have no idea what happened, but I know why. ME2 is remarkably glitchy. I think it was a combo of the inferno grenade, collector lazer, harbinger singularity and my own singularity that actually killed me. Everything kind of culminated and shepard exploded. His body shot back at least a hundred feet. But his head stayed there. It cut to the mission failure screen and there was just an armored shepard head that had been contorted due to force that was just leviating above a chest high wall.
3. I was leveling a DK in wow for the 5000000th time because i was bored and made it to the final quest. I had gottn there just as the cutscene started and one of the npcs was feeling especially...zealous. It just so happened to be the one with the 1000 yard radius. He constantly cried "Stand fast, brothers and sisters! The Light will prevail!" as he hacked down fledling death knights attempting to retrieve their bodies. I managed to escape his wrath when he was up on the graveyard killing a group of freshly spawned knights.
