One of the fundamental laws of the universe is that no system is perfect. A single overlooked flaw can cause the entire house of cards to come crashing down. This only becomes more apparent when the system receives input from outside. Even digital systems are flawed, and can have bugs inherent or introduced that make everything stop working.
When the Super Smash Brothers successfully held off Subspace, Sakurai had thought the system would be perfect, but there were further complications. Even outside introducing the new fighters, a single bug in the system's core began to grow and multiply, until the entire world had been remade in its image.
It was almost too late when Sakurai and Miyamoto found out.

"This is bad," Sakurai says to himself, over and over. "If this can't be contained, it could spread to the entire company... or the entire world." He pulls up a chair to the computer.
"Have you gotten back into the system yet?" his colleague, Miyamoto, asks. Miyamoto is still standing up, and though he looks calmer, he's still fidgeting, betraying some worry.
Sweat runs down Sakurai's face as he frantically taps on the keyboard, trying to work out a way in. "Nothing's responding! All I can get is the trophy room, but everyone's been deleted!" He suddenly gasps, and Miyamoto looks over his shoulder to see why.
[RECOLLECT TROPHIES?]
Images flash across the screen, but none of them seem like characters that Miyamoto had designed, or that Sakurai had known of before. "The Board is going to kill us if they find out what we're doing," Sakurai says, apprehensive.
With a solid hand on the younger man's shoulder, Miyamoto replies, "But if we don't do it, then everything we've worked for will be destroyed. If we do this, then there might still be a chance."
His resolve steeled, Sakurai presses the 'enter' key on the keyboard.

The screen gives out at that moment, blinking into nothingness. "Is it still running?" Sakurai asks, his panic still evident.
"Leave luck to heaven, my friend." Miyamoto leads Sakurai out of the building. There's nothing more they can do except hope that the new trophies will reclaim the world from the bug, and that they'll live long enough to see it.
