System Shock 2, when you find out that
Dr. Polito was dead, and that you were actually being "helped" by SHODAN.

Oh God, that took any sense of accomplishment you had worked up to that point and punched it in the throat.

S.T.A.L.K.E.R.: Shadow of Chernobyl
The actual reveal was delivered poorly, but when your identity is revealed and you're told why you were originally programmed to kill Strelok, it all is incredibly tense.

In NMH when
you find out henery is actually your brother.
What the hell? Why would bring up something like that at the very last minute of the game?
Why would you open spoilers for a game that you haven't beat yet?

Edit: Whoops, misunderstood your statement.

Did not expect it one bit. I just thought it was Atlas' dialect.

But BioShock beats them all. God damn that was well done and interesting.
Thanks to the Master Miller reveal in the first MGS this never impressed me, I just thought to myself "wait, did they just do a Master Miller, really?"

But I guess that the unfortunately side effect when you play a lot of movies or games, can't tell you how many times I've seen this plot twist in films.

Jak 2 and Jak 3.

The people who've played them will know what I mean.

Planescape torment BIG SPOILERS, plus some speculation. You have been warned.
When you find out that the reason you are immortal is because your first incarnation did a crime that was so horrible, there was no way he could make up for it in one lifetime. Now, the crime itself is not revealed to the player, but one of the more popular theories is that you started the blood war

For recent games, I've got to go with the Left 4 Dead franchise: we recently got The Passing DLC which indicates that my favorite character, Bill, has become infected or something and become a creature very similar to the infected in the game's universe. Hopefully we'll be seeing the expected L4D1 DLC come out soon that will explain how that happened.

BioShock, The World Ends With You, and KotOR.

Also, the MGS franchise. Though I've always been wary on the plot twists of the franchise, because sometimes they take it a tad too far. MGS did a pretty good job. MGS2's ending made my brain have a minor implosion. I'm freaking serious. I couldn't think straight for days.

MGS3 did it better, though... Having a good plot twist but it doesn't overdo it. It's not as big as any of the other games, though.

MGS4 /almost/ went too far, but it landed about a centimeter away from going overboard with it. So I thought MGS4's was perfect. Though my personal favorite twist of the franchise is the one(s) in chapter 3 of MGS4.

Bioshock. That's all you're getting. If you've played it, you already know exactly what I mean. If you haven't, then I'm not about to spoil it here.

By the way, is there anyone on this site who still doesn't know the twist in Bioshock?

If so....get on that. Seriously.

Mass Effect had some pretty fantastic ones. Although if you could count the ending of Red Dead Redemption as a plot twist, that's the one that floored me the most. Mainly because the excellent twists in KOTOR and Bioshock had both been spoiled for me before I played them.

Metal Gear Solid 3's ending. The plot had been relatively straightforward and simple until WHAM! Everything you knew was WRONG!
