Fallout 3's Point Lookout for starters.

How would you react to finding out someone surgecally removed a lump of your brain? yeah, I thought so.

Also, Fable 2, after recruiting your last Hero, Reaver, and begining the summoning of the 'weapon' that will defeat Lucien, only for Teresa to disappear, Lucien to appear with Giant Shards and Commandants, captures the 3 Heroes, tells you that your family's dead, shoots your dog, and shoots you....again.

Heck of a way twist a plot, think you got it all knocked down on the quest for revenge, only for it all to end up seemingly screwed over.

Bioshock no doubt, that twist was something I'm certain no one saw coming.

Heavy Rain's also had me a bit surprised.

(Note: Spoiler for both games)

Scott was not the guy I expected at all, he seemed like the least likely candidate in fact. Although, I din't expect Atlas either, both seemed like genuinely good guys trying to right the wrong done by others.

I know its been done to death already, but for me the biggest plot twists have come from Bioshock and KotOR. But I can remember one other plot twist that stuck with me from Final Fantasy X

In the late parts of the game you discover that your main character and Auron are already dead spirits who havent moved on yet.

Suddenly it detonates while still in orbit, sparing the people on the ground from nuclear fire, but disabling all unshielded electronics with a high powered EMP. The insuing chaos of helicopters and jets falling from the sky allows Ramirez's squad to flee to safety and retake the White House from the Russians.

In short, PRICE SAVES THE DAY!!!

Even better than the discovery that the Collectors are just the Protheans after the Reapers enslaved them comes in the final moments of the Collector General's life.

I assumed that the Harbinger (the entity that repeatedly possessed the Collectors you fight) was the Collector we see in their control room during the rescue mission on Horizon. He always said, "Taking Direct Control" whenever he arrived and "Releasing Control" whenever you killed him in the field.

Only as the Normandy is fleeing the Collector Base in 'Suicide Mission' do we see that Harbinger is actually a Reaper out beyond the galactic edge communicating/controlling the Collectors in real-time. He even says "Releasing Control" just before the Collector General dies in the explosion.

I was so pissed when I realized the bastard wasn't going to die on that base with the Collectors. He'd been my nemisis through the whole game and I didn't even get to kill him... one simple word encapsulated that moment for me: "Son-Of-A!!!"

But we get to see him coming back for more in the final seconds of the game... and he's bringing friends:

I'm sooooo glad you said this! This is my favourite twist in any game EVER! My mind was just reeling over it for days afterwards.

Oh and the game is called Second Sight, it's made by Codemasters, the guys that do Timesplitters and it is awesome!

Thanks to the Master Miller reveal in the first MGS this never impressed me, I just thought to myself "wait, did they just do a Master Miller, really?"

But I guess that the unfortunately side effect when you play a lot of movies or games, can't tell you how many times I've seen this plot twist in films.
I never saw the Master Miller thing coming, I felt soooo stupid. Even when I play that game now I'm like, "HOW did I not see it?" Then again I was much younger back then!

Too be honest the whole Card Key thing amazed me (and pissed me off), totally didn't see that one coming, such a cheap trick!

Unfortunately I also see quite a few plot twists coming these days, it takes an extra special game/film to really impress me. Or just a really cheap shot... o_O

I got the twist for Shutter Island before I even saw the film. I went in with a pretty good guess. By halfway through I already had all the proof and even worked out the anagram. It was amusing to hear all the people talking about how they didn't see it coming when they left the film!

Oh come now, you can't tell me that you honestly didn't see no. 2 on your list coming? It was a totally obvious Call of Duty thing to do. Besides, when someone is given no identity other than some sort of codename, you know there's a reason why. For example, Mass Effect 2:

Take Mass Effect 2, it didn't work because of all the pre-release media surrounding the character, but play the level in the prison space station where you have to get "Jack". Notice that NO ONE refers to Jack as anything other than Jack or Subject Zero, they never say he or she. This isn't natural and is obvious from the outset that they mean to cover her gender. The reason? Because Jack is a male name and they want it to be a surprise when you see a woman. Oh they are so easy to see through!

Did you guys ever play Second Sight on xbox? That was a great ending ( the scene with what I consider the last boss in the office building at the end, just before you end up in the cave helping the psychic kids ).

I like the one were it turns out the real enemy was a jar of pickles that mutated becuase of space radiation, but after your done defeating them ie chowing down, it was all a lie to get you to eat explosives. Thankfully you kept one last pickle becuase you were going to take the evil throne once you won and so you chase the villian around trying to get him to eat it and finally when you do you wake up the day before you invaded the castle.

ok that really didn't happen but that would be sweet if someone did do that. but heres the real one
in Nier towards the end you have to fight your to twin friends from your village becuase they were secretly helping the villian and later you learn, that humans have been extinct for over a million years, and in a last ditch effort to preserve humanity, mankind split body and soul. so all along you've been playing husk nier and when you play through it the second time, it shows you that the shadow king was actually the soul of nier that you played at the beginning of the game. along with that it shows you just how big of a grey area your in by showing the shades stories. its...so...fucking...awesome!

Dead Space's twist kinda had my jaw-dropped. After all that, Everything just goes to hell. xD

Final Fantasy 13's revelations were kinda baffling to.

For me, its a tie between No More Heroes (slow down the sped-up part (I think theres a Youtube video somewhere) and the end were two swift kicks to the nads) and, of course, Bioshock. Modern Warfare 2 caught me off guard as well. I wouldn't say it was the best, but damn it made me jump. Oh, Dead Space's was pretty good too.

Yeah, this fucked me over, I thought those guys where dead but them BAM I was wrong.

And the twists in MW2 where good as well.

I can think of at least two games with that kind of twist:

Heavy Rain and Persona 4

Ok, KotOR twist was mentioned by hundreds already, I still didn't finish Bioshock so I avoid spoilers like a fire, and Persona 4 twist, while very interesting, I managed to guess the killer somewhere after Kanji dungeon, mostly thanks to the 'laws of making video game villain'
1st law: if the identity of the villain is secret, it has to be someone you encounter frequently.

2nd law: if the case is a murder, the killer probably has some connection with the investigation going on.

3rd law: do NOT trust the goofy ones or extremely nice ones, because often that's just the mask

And so, since
Adachi
was a perfect hit, instead of a shocking twist I got 'Hah! I knew it' reaction. Still, a great game.

Anyway, Heavy Rain twist mentioned in several posts above was the one that genuinely surprised me, even if I SHOULD guess it from the very same laws of making video game villain I posted above, but I didn't thought the creators will make THAT kind of move and
give you the control of the main villain without your knowledge

Apart from that, frompably Fate/Stay Night:

Archer is you from the future who became hero just as you wanted, and he's not happy with that... at all

The game dropped a few hints about that, and I guessed it about 5 minutes before the revelation was declared public, but I was like 'Oh shit... Don't tell me...' (*they told me*) 'Shit, that's awesome'. Everything suddenly made sense, and the game became infinitely more awesome after that.

Heavy Rain's twist pretty much floored me...

Though in some ways it lacked a bit of consistency in the lead-up to it; various character thoughts, even actions.

Also; Tony Hawk's Underground. Totally.
