Well I'm currently playing darksouls so these guys.

Fuck these guys.

"this land is peaceful, its inhabitants kind"

Riiiight, I can tell by the hollows, wheel skeletons and dudes impaled on spikes everywhere. Forcing me to fight those damn things is reason enough to kill Priscilla.

I often play melee-oriented characters, so...
Psh... get a decent skill with guns and a sniper rifle and Cazadores are easy pickings. But I can see how they would be a pain for melee characters.

My personal favorite (and by favorite, I mean I hate them with the passion of 100 suns) are these guys...

Seriously, screw reavers.
Ahh fallout enemies ehh?

Well as much as I dislike those, I'd have to say for sheer scare factor and frustration at getting killed when careless, you gotta go with a classic.

I really don't play CoD much at all but the few times I have... those f*cking juggernauts. They just don't die! =O

Aside from them, these little f*ckers;

"Aww, look at how cute you are! With your wee hoody and lantern! ^^ And what's that you got there? Is that a kni-" It's killed you by this point. And it enjoyed it. A lot.
At first I also found myself hating juggernauts, but I found a vid that taught me how to one hit kill them. Granted it takes some skill and practice but oh boy is it satisfying to kill one of those motherf*ckers with one hit of a grenade to the face and see them drop like they were nothing.

I didn't dread tonberrys since I actually enjoyed them. Whenever I saw one in FF8 I was like, OH SHIT better kill this dude quick, the encounter simply felt exciting.

The one enemy that I did dread back in FF8, back when I didn't know how to properly junction against this particular enemy, was Malboro. Christ, bad breath and its over.

Nothing really these days but back when I played Ocarina of Time... the dreaded ReDead. I was a kid at the time and their freezing screams followed by their creepy constrictor move legitimately scared the crap out of me.

I hate Oblivion zombies. I dreaded going into dungeons enough without those guys. I'm not sure why since they aren't hard and not that particularly gross, but they're just so... organic looking. I'm fine with all other types of undead though.

Also from the very first moment I saw mutons in the new XCOM, I never wanted to see another one, so naturally they replaced Sectoids as the most common enemy encountered.

Dear god yes let's ***** about the Bethesda Fallouts. I'll see your Deathclaws and raise you some Giant Radscorpians. Deathclaws are terrible, yes, but they have heads, which means you can shoot them in the head. And you can also cripple their legs at range, negating their leap attack. Now radscorpians, on the other hand? No weak spot, meaning you have to deal with their armor at all times. Crippling their legs doesn't affect moving speed, crippling their pincers won't stop them from cutting your goddamn head off, and if you weren't paying any attention to their tail? I hope you like poison, 'cuz you're about to get a metric shitload of it.

The best part is that they're all over the fucking place. You can't sneak away from them, so if you aggro a group of them by accident get ready to fight all the radscorpians.

...Wait. What the hell am I talking about?! Nightstalkers. Fucking hell, Nightstalkers. Let's run down the checklist, shall we?

- Always hissing like snakes. Always.

- Always found in groups of "three" to "fuck your couch"

- Goddamn creepy snake heads

- More HP than they deserve (they don't deserve any HP, the little shits)

- Extremely fast, coupled with a habit of getting right into your face when crouching. "Oh crap, they spotted me. Better go into V.A.T.S. andOHAI HOPE U LIEK POISON LOL"

- do more damage than anything even remotely coyote should, because of poison damage

- Bullets randomly pass through them, doing no harm. Setting everything else aside, that is just goddamn cheating. Whatever programmer allowed this to happen should be strung up on Bethesda's front lawn, flogged within an inch of his or her life, and then left there as an example to the others.

- Those lovely episodes wherein the local Nightstalkers are invisible from chewing on stealth boys. It's like "Yo dawg we heard you like invisibility so we made some Nightstalkers randomly invisible so I hope you like poison."

I HOPE YOU LIKE POISON

Seriously, fuck Nightstalkers.

Bethesda has only made one Fallout game: 3. Obsidian (a company made up of a lot of the people who did the first two games) made New Vegas, albeit based heavily on Bethesda's work with 3, and published by Bethesda, and rather critically, using Bethesda's QA team.

Lately I've been playing a lot of New Vegas with jsawyer.esp, a balance mod created by the lead designer of the game, which (among other things) drastically reduces HP values. Poison can get really overwhelming really quickly with that mod. Poison immunity is the one body part perk I didn't even have to think about keeping after finishing Old World Blues.

EDIT: Also, for as much trouble as cazadores gave my melee dude, feral ghoul reavers? Not really an issue. Super slam trivializes a lot of fights.

Kerbal Space Program.

I dread the Kracken. You turn on your time warp to finish an orbital maneuver quickly, and every now and again it will glitch and your rocket will explode.

Oh by the Moon, you nailed it; the marsh horror was horrible indeed. Me and mates I played this game with, used to call her "flower pot" - she'd plant you in the ground if you allow yourself a split second of hesitation.

My personal Most Dreaded Enemy are the bloody damn monkeys from System Shock 2:

You'd hear those things waaaay before you could see them... just scurrying there in the dark, making those awful monkey sounds to themselves... and then they'd jump out on you from nowhere, screeching like a banshee and hit you with their genetically implanted psi power. Ugh. Them having top part of their skull removed, brains visible, did not help either.

And then there were the mid-wives... actually, I'm just gonna say everything you meet in SS2 was pretty damn terrifying.

Okay, I'm new to Dark souls. I'm in the Darkroot garden playing as a tanky motherfucking Knight. No enemy has one hit killed me EVER. I walk up to a tower and there he is, standing JUST INSIDE the doorway to the right, Havel the Rock. I'm like "Pfft, stone ass dude with a hammer, no problem". He prepares a swing, I put up my shield and BAM! He one hit kills me THROUGH MY BLOCK!

See that thing on the right? That's his shield! IT'S AS BIG AS HE IS!
Oh you haven't met the Capra demon yet, have you?

You'll see... You'll see.

EDIT: Havel is ridiculously slow. Just circle him.
Aye, what I'd like to add as my contribution to this thread is this:

EVERY SINGLE ENEMY OR BOSS in Demon's and Dark Souls. The first time you bump into something new, you better be alert and have all your wits about you. The first time you confront a boss type foe in these games, you're bound to get proper scared (and your bottom spanked). The first time you know that stepping through some fog gate might mean it's boss time business as usual, you'll grow rather weary with stepping through fog gates. What a relief when it's just another area, and not some huge big thing intent on killing you!

Yeah, Havel scared me proper. Just standing there, and then he runs at you and plonks that dragon bit souvenir at your head, compacting you like a dead car. The first time a surprise Taurus Demon drops on your head. The first time you go over and try to have a cup of tea with Mr. Capra Demon. The first time you see something looking like... an armoured boar? Then there's plenty of assorted foes you do want to mess with, but don't want to get cornered or backstabbed by. Giant mushrooms. Oogly frog things

I think to be proper scary, the foes have to look intimidating for the very first time you encounter them, and have some special trick up their sleeves so repeat encounters don't get too boring and mindless fun. Make a mistake with the big mushroom people, they'll bash your head in. Neglect the cursed furby frogs, they'll cause you to curse alongside with them.

I agree that just watching the giant eyeball frogs do their thing is disturbing already, seeing them come after you is well unpleasant... and I think that's awesome! In Dark Souls, you end up cutting up 8-ft-tall knights like sushi, you pummel and backstab and outright toy with huge cow and goat demons, but little froggies will still curse and kill and bother you if you disrespect them. Awesome.

Kobold Shamans in DDO are a nightmare...

Not only do they put out some serious hurt on lowbie characters with their spells, but they have crowd control effects (Higher difficulties can mean one stun landing gets you mauled apart by the swarm of kobalds around you), they have more health than other kobolds (Even higher than the melee ones) and to top it off, they'll jump backwards if trying to melee them (Often they'll jump up a wall and be out of reach for melee)

I'm sick of dredge in GW2... Not only are they all immune to blind (They're moles... They don't see) which is a major source of damage reduction but each type has annoying traits:

Ratniks do high damage consistently, fast enough that it can be dodged often but slow enough that it isn't easily mitigatable burst damage.

Disaggregators provide high uptime boons of Swiftness (Can't kite them), Might (They do more damage) and Protection (Can't deal direct damage very well) to all enemies in an area. Oh yeah, they also daze you so you can't use your skills for a few seconds.

Resonators do a line damage hit that if in melee range hits multiple times, they also do PBAoE knockdown (Though is telegraphed well and can be avoided, unless in a big fight and pets/illusions can't avoid it)

Oscillators knock back people in melee, which gets frustrating...

I think for pure fear of getting murdered it would tie between the Cazador and the Wheel Skeletons in Dark Souls and possibly the Raptors in Dead Space 2. As for annoying I would say just any stupid flying enemies like the Harpies in GOW but by far the most annoying enemy I have fought is those fucking Incendiary Dagger Dogs in Ninja Gaiden 2 I guess I dread running into them too them and the Basilisks in Dark Souls.

SlaughterFish in morrowind sure cliff racers are the most annoying things but Slaughterfish are the most scary in the fact that they sneak up on you every-time you swim, The sound of their bites hitting my heavy armour is so freaking loud

You can never be properly prepared for Red. Ever. The moment you first speak to him at the top of Mt Silver you realise you have made a terrible mistake.

Skeletons in Dark Souls.
You arrive in Firelink Shrine, decide to do a little exploring and BAM!
You come across these guys:

You manage to kill them (barely).
And then they re-assemble themselvs and kill you.

The golden/red mine laying submarines in Sinl Sub Pro. They are no tougher than any other enemy, but they do release these fucking tiny floating mines which are a pain to destroy.

These Bastards from Homeworld 2:

No biggy, they are just frigates right?

WRONG!

3 of these suckers could wipe out most capital ships, let alone a fleet of 20 in wall formation

Fuck those frigates man. Those are missile frigates right?

The most annoying thing about the Vaygr is that they have the same frigate chassis for like 3 or 4 ships, so even if its not a missile frigate, you still see the same outline all the time.

Also the Vaygr love their missiles, seriously... they could be far away and they launch their missiles and you just sit their and watch the slow death approach your ships, so frustrating.

Yeah, Heavy Missile Frigates! Bastard things... You can't even outrun/stop them, sometimes i've had battlecruisers with full health slowly get blown to bits as they try to fly away. And to add insult to injury, the Hiigaran Torpedo frigate is incapable of doing anything.
