Probably ninja gaiden, though maybe it was just because I was really young when I started playing it.

Europa Universalis 3, everything set to hardest, no lucky nations and I conquered the entire world as the Inca.

Call of Duty 2 on Veteran. I remember it being very, very hard. Also, I'm 17 so I missed out on pretty much anything pre-PS2 (including all the really difficult classics I keep hearing about).

Incidentally, my first post on this site was about how I'd just completed that.
I would say that's the same for me. It took so long!

The distinction of the hardest game that I ever completed will have to be shared between Super Ghouls and Ghosts and Ninja Gaiden(NES), though most of the challenge in those two games was due to cheap game design rather than any legitimate challenge.

Both Bayonetta on Non-Stop Infinite Climax Mode and Ninja Gaiden Black on Ninja Master mode felt more fair in their difficulty than those old school challenges. Maybe I should pick those two instead?

"Sergeant Randall! The guns are gone!"

"What?"

"The-guns-aren't-here! The krauts must have moved 'em someplace else!"

"Well hell, keep moving!"

*Blamblamblam-urrrgh*

...

"Sergeant Randall! The guns are gone!"

"What?"

etc.

Original Contra, no cheats.

Man we came out of that game shaking like we'd just down a few No-Doz with a couple Redbull chasers.

Tossup between ADOM, Nethack, IWBTG, Dungeon Crawl Stone Soup and Perfect Cherry Blossom (phantasm stage thereof).

I finished "I wanna be the guy" about a year ago, but I am going to start "Demons souls" which my brother finished, but not until he nearly threw his controller out of the window in frustration. But after I completed "I wanna be the guy" I feel like I can do almost anything.

My condolences. IWBTG is... amusing, for a while, but definitely not worth it at all.

The Journeyman Project: Legacy of Time.

Fun Myst-like adventure chasing clues via time travel through El Dorado, Atlantis and Shang-ri La trying to find the secret of an alien civilization that visited Earth and is now returning.

Played it with my dad and it took us months to finish (partly due to complexity and partly due to only wanting to play together). In the end, we got stuck on the final puzzle and almost stopped playing the game entirely. I ended up beating up one day by clicking the puzzle at random and then TA-DA! My dad was so upset that I beat it without him and couldn't figure it out again so he could see the ending.

Final Fantasy IV on the SNES. Not because the game was itself hard, but because I was trying to beat it via rental. Six months of battling douchebags deleting or messing with my saves, having to restart the whole game practically every weekend...I finally got so fed up with it that I rushed straight through, got to the lunar core twenty levels below what I should have been and spent twelve hours straight grinding to get the absolute minimum level to beat Zeromus.

That was the last time I ever rented an RPG.

SIReN: BLOOD CURSe.

R.I.P, Ps3 Controller No. 3 (whom I nicknamed Bill). May you have peace in your eternal slumber of being snapped in half.

In recent memory? Alien Swarm. Me and my friends beat it on a dedicated Steam server about a week ago. By the end, everyone was attempting to burn their computers and murder Gabe Newell.

Another one: Killing Floor. I played this on solo; I beat the first couple of stages -about three- and was completley oblivious of the fact that I realised I should of stopped playing before I murdered someone.
