Difficult to say, but ALS and Altzheimers would be an horrific combination. It's really dependant on what you personally are most traumatised by, then taking into account duration and acceleration of the illness. Paranoid Schizphrenics can have an intolerable existance, depending on the severity of hallucinations.

As some people have quipped, Ebola is also an horrendous disease, but it tends to have the good nature to kill you quickly. However you can be infectious for up to a month before symptoms begin to fully develop. So it's scary.

I've been terrified of Ebola ever since I saw the movie "Contagion." AIDS is pretty bad too, though
