Depending on the scene certain physical conflicts, the main one I have a hard time watching is the Assassins Creed cut-scene where the prisoner gets his knees broken, this is painful for me because I've had major problems with my knees and I know how painful it can be. Other than that I've got an overactive imagination so sometimes watching something happen I'll feel "pain" it's more of a pressure but I can imagine what it would feel like.

Being armless, not having no arms, but having no weapons. When you're totally vulnerable. I hate it, i like to have at least a stick to fend off foes with.

Nope, unless you count effects that mess with your sight like putting on the (stupid fucking god damned) Big Daddy helmet at the end of Bioshock.

Other than that sort of thing, probably the knowing that there are enemies nearby, when they know where you are without you knowing exactly where they are or when they are going to attack, which kind of makes me go extemely paranoid, jumpy and with that sinking feeling of how you could die at any second.
You know, you don't HAVE to kill him right. You can spare him and still find and rescue shaun, and get his good ending. So you're not really forced to do it.

i think the one exception to that is Uncharted 2. it actually looked pretty realistic

something that makes me uncomfortable in games is when im playing online and i KNOW that someone on the other team is deliberately hunting me down. like when you die to the same person over and over again in a variety of different locations and never die to anyone else. it makes me feel like im being stalked

I was shooting. It's just a game, I thought, might as well play it.
Made me uncomfortable though, probably due to the sound.

Another thing in New Vegas...
I followed the Wild Card questline with Yes Man, mainly due to the fact that I wanted to get a proper robot army set up, then use it to assist the NCR in bringing order to the Mojave.
However, at the end, the only things I could say was:
"Piss off, NCR."
"This huge robot army wants you to leave."
and
"Kill all NCR in the area."

When I wanted to say:
"I've brought reinforcements."

That pissed me off, mainly due to that not being what I wanted. I did not spend the entire game helping the NCR where needed, then backstab them and boot them back west.

I'd say either the "No Russian" mission in MW2 or playing in multiplayer on any shooter that has a .50 caliber weapon and its never an instakill. It's a .50 Cal! No body armor in use today can stop that! Doesnt make me physically uncomfortable, but pisses me off.

He has a giant hammer, knocks you down, and crushes your face with it.

I know the feeling, even tohugh I often find myself testing it to see if the designers put something horrible down there.

Whiny annoying characters, a la Shion from Xenosaga... oh wait no you mean physically uncomfortable not just mad as hell. Well it was uncomfortable watching her fawn over some douche bag and in the third game watching her berate Miyuki... who's only ever been helpful what with designing better and better weapons for you.

Dumb ****. Man, just thinking about her makes me raaaaaaaaage!

Also, being a jerk for no reason other than to be a jerk in games makes me uncomfortable. Unless it's funny like Saint's Row 2. But in ME2 being a renegade sometimes was hard for me.

I'm pretty twitchy while playing Amnesia, apart from that no physical reactions.
In a sense I hate playing asshole characters, though. Whether it's the character itself (Rubi from Wet comes to mind) or scripting choices (like in a Bioware RPG), I always feel uncomfotable being an asshole. I've played a Renegade charcter in Mass Effect 2 and sometimes I had a hard time bringing myself to click those red words on the dialogue wheel.
And for just general uncomfortability, BioShock after "Code Yellow" (hell, it starts at the wyk bit). I just get damn depressed every time I get to that part. I'm still in shock over that twist, I think :|

Well, the thread title asks for something that makes me physically uncomfortable, so despite the pages of mental discomforts people are listing, I'm going to answer that question.

Any Wii game that relies heavily on using the pointer (usually shooters). I can't even finish the first level of Link's Crossbow Training without my wrist being in great pain.

Fucking creepers. The ones from minecraft.

Its always the same story most of the time- see a frown run like hell or shoot it down.
But those glitched creepers... they just sit there, head slightly down, either waiting for the end or for you to run. It is just...sad.

The Invincible, Regenerating Hunter in Dead Space 2 that chases you for two whole fucking chapters, the unbaptized babies from Dante's Inferno (that soon met my whirlwind of a scythe, no less), and Alma from F.E.A.R.

when you find your 100+ hour save file has been corrupted

the sound of falling death in Mirrors Edge is pretty nasty as well, as you see your hands wave about and can see yourself falling.

I don't think "scary" is the word I'd use.
