The golden claw in Skyrim is the most recent in memory.

I don't know if anyone actually said "the answer is in the palm of your hand" or anything like that... but I just smacked my forehead at this when I figured it out.
Yeah, the journal you loot from the jerk that had the claw says that exact thing :p

Dragon Age 2.
During act 2 when you get to fight Xenebeck.

HOLY SHIT that was a lot of very large very angry demons that popped up.
Could have warned me that is was not just a BOSS but a boss and every single mook demon for 10 bizillion miles.

Just had a minor one in Skyrim, actually. After getting hounded by a second random dragon for miles, I decided to go back to the main quests until I can at least learn the first full dragon shout with the greybeards at High Hrothgar.

Except, I really wish I knew that there was a village that had a massive staircase to go there in an easier manner than spamming jump on a whole lot of steep mountain. I found the location of said village on the Skyrim wiki recently though.

On the bright side, at least my leveling during my big tangent quest has probably prepared me for the Frost Troll that's apparently nearby.

In KOTOR 2, there is a part where they force you to fight as Atton Rand, but I had never levelled him up, as I didn't like him and hadn't been told that all companions needed to be levelled.
Oh yeah. It took me forever to figure out I all I had to do was hide behind the bar and shoot them. That had me raging for a good bit.
I levelled him and equipped him with some good gear and it still took me five tries to kill those twins and it only happened when the game glitched out and they got trapped behind some furniture.

Crysis 2. When the capture Alcatraz or first meet the scientist, a simple, "Im not the nanouit solider youre looking for" would have sufficed and saved a few guns to the face or thwacks from a gun butt.

I was playing Left 4 Dead with some random folks once and we were cutting through some buildings when we began to hear the Witch crying somewhere nearby. Undaunted, our fearless defacto leader proceeded into the next room, an unfinished room of drywall and hopped on to a workbench to give us some sort of instructions.
Unfortunately the Witch, who had blended in to the drywall and the bucket she was crying next to was on the table. I tried desperately to warn him that he had just jumped next to a crying killing machine but all I managed to stammer out in the seconds he had left to live was, "Uh-um-uh-uh-um!"

*Shows a link to your post....*

I guess you do have to say more :p
My bad.

Theresa.
Theresa the Seer...

I mean, "Hey! I... y'know, saved the kingdom from my d*ck brother, who suddenly turned into a d*ck a few years ago"

Theresa - "Hey... there's a massive evil gonna destroy the kingdom if you don't have 40 million gold pieces..."

"So... my brother was being a d*ck so he could raise enough money to save everyone in the kingdom's life?"

...

This one boss in The World Ends With You, pretty sure it's the first thing to be actually resistant to anything and as luck would have it all my usual pins, my partner's pin and Fusion for god's sake, does zero damage. Stupid beetle thing

New partner? Make next boss immune against him. Genius guys, absolute genius. Maybe it's making the new asshole partner seem like more of an asshole by making him worthless? Maybe? In any case, can i haz Shiki back nao? Actually had to lose on purpose since i'd been playing for 15 minutes and i hadn't scratched it.

I was recently playing Serious Sam: TSE, where I was in a fairly large arena filled with kleer and those giant two-legged robot things

I finally kill them all and then I see, in the middle of the room surrounded by a wall, the cannon

I literally went "you could've told me, game!"
That was quite egregious in BFE - the Devastator is quietly lying around in some corner halfway through the map on two seperate occasions.

Yahtzee's recent review sounded awfully joyful about
the jetpack
but I thought its placement equally ridiculous. THE vital component of the battle, hidden in some hut while you have 4 seconds to live and figure that out. WTF Croteam, WTF!

actually it's not all that hidden
I mean, it's on a large building to the right from where you start and you will most likely go there at first as it has a lot of ammo and armor shining right there to attract you =P
I didn't grab it at first because I was hauling ass from the motherfucking barons of hell guys and thought it was armor, not a jetpack

Wrong. It's in an enclosed building*; you can't spot it right away. What you do spot right away is the tons of cannonballs, large healthpack and rocket crate to the left - you know, completely opposite of the jetpack.

And that's terrible design in a boss fight that kills you after 2 seconds on the ground.

(*until it gets blown up, but chances are high you blow up with it)

In the cave looking for Alduin's Wall in Skyrim. After solving the first puzzle, you get to a room with a bunch of symbols on the floor. When I got to the room, the NPCs got up behind me and said nothing, so I ran into the room, stepped onto the wrong symbol and got pwned by fire. When I got back to the same room the NPCs explained that some of the symbols were pressure plates and by looking closely I could navigate the chamber without setting off a trap.

Final Fantasy XII. At the very beginning of the game there are some chests you're not allowed to open if you want to ever be able to get the Zodiac Spear, which is basically the best weapon in the game. I've only found this out once I was almost at the final battle.
