Why is the main character the chosen one? Just for once let me play as some totally average dude who has to think to survive?

There are crates. Everywhere. EVERYWHEEEEERE!!!
Not only that, but if you decide to break open said crates, there will be things inside to help you. (It is perfectly acceptable to replace crates with pots, boxes, barrels, or what-have-you.)

Also, almost every crate/box/item-storing-device is labeled with big pictures of what is inside. Is everyone in video games illiterate? Can they not just write what's inside on the side of the box?

Spunky black sidekick.
Disapproving wet-blanket love interest.
Main character with a sketchy past and nothing to lose.

In any and all JRPGs, the You Couldn't Sleep Either Scenario.

Here we have main character boy and supporting character girl. The two have built at least a decent romance subplot between them, but at least once in any game the following scenario must occur.

Boy wakes up in the middle of the night from troubling dream. Boy leaves area to go to some scenic place, usually a seaside cliff or balcony. Either boy finds girl standing there as well, or vice versa. Either way, the following line is uttered by one or the other:

"So, you couldn't sleep either, huh?"

These two will fall in love without fail soon after this scene concludes.

On another note, it makes for a very effective pick-up line. Seriously, only twice or so have I not picked up a new friend or a date from asking that in the right circumstances.

Red barrels = Explosion
Also, all enemy strongholds are required to stock dozens of said barrels at regular intervals, and elite special ops soldiers are trained to take cover behind them in the event of enemy attack.

And no matter what the game, the bad guy always feels content to throw the most incompetent soldiers at you first, arming them with little more than a pistol, until you've gotten used to killing them. In fact, their soldiers are often arranged in a strict hierarchy of skill and color coded, so they know exactly who to throw at you at what point in the game.

In FPS games - Grenades that bounce all over the place, weapons that have unacceptable recoil and mines that insta-kill you no matter what you do.

well I don't know if grenades bounce(I suppose they do) but umm...mines DO kill you, they might not kill you instantly, but I don't think it would be entretaining for the person gaming to trip on a mine, and seeing as his player is dragged into the hospital and has to walk on a weelchair for the rest of his life is it?

Walking onto a medical kit that instantly heals half of your stamina, despite no actual applying of medicinal objects.

How about this: You need keys to open doors when you have a bazooka. Like in 007 Nightfire! I need to find a key to unlock a sliding, thin door when I can shoot the lock?

Shoot in the leg= You can walk just fine.
Shoot in the arm= You can still hold things fine.
Aside from obvious ambient lighting, all glowing things are useful.
More muscles mean you can take more bullets to the head.
Electronics explode when shot
You can not shoot your way through a wall/board but 3 guys at once is fine
No matter how hard you try, you cannot see up skirt or down blouse.
Clothing is semi-permeable to bullets
There will be a perfect opportunity to use something right after you get it.
You can never use a skill ever in your life, and still be getting better at it
Rockets blast huge metal doors open, but will leave trees/boards/walls in tact.
You can instantly remove ammo from weapons laying on the ground
You will NEVER drop a gun
All doors are bullet proof, and no amount of strength bullets rockets, explosives will budge it. Holding a key will let you pass though.
NPC's will still be there once you get back
You will ALWAYS fight bad guys, even if you are bad, you still fight other bad guys.
If its pointless to go that way, you cant.
You do not feel pain, and it will never effect you.
Time will freeze till you get there.

I'll think of more later.

Adam Smith hates your guts.

It doesn't matter how much danger they are currently in, the NPCs will never give you a discount on anything.

Treasure chests, just lying around waiting for you to open them up and steal the gold. Didn't they have banks back then? At least try to hide yr damn chest, why the hell would you leave it laying around in the forest anyway?

Biggest Cliche... That even under heavy fire and when the enitre military platoon you are with is swamped with missiles, that the four characters you happen to be controlling are able to escape without death, and then go on to fight the entire alien army themselves (Yes I am talking about gears of war)
