What do you feel are the scariest elements of games?

I mean, who are the scariest characters, what are creepiest locations, the most bone-chilling music, etc. ?
Also, which games tried hard to be scary but ultimately failed?

For me, the Shalebridge Cradle from Thief: Deadly Shadows was just... *shudder*. Wallmasters (Ocarina of Time), too - horrifying!

Practically the whole of Yahtzee's game 5 Days a Stranger (seriously, if you haven't played it, play it now)
Practically the whole of the rest of Yahtzee's Chzo Mythos quadriliogy - chase sequences with a welding-mask-wearing, machete-wielding, murderous, vengeful spirit :O ; also the transition between the normal and spirit world in Trillby's Notes :O <<

For me it would be pretty much everything in the suffering,not played it?

I can remember the opening to Bioshock where you get attacked in the bathesphere, a bit tame but I was very reluctant to get out of it

Dead Space was by far the scariest game I've ever played. The suspense of not knowing where an enemy would come from is scary in itself but what really got it for me was the background music. Every once in a while you'll hear people talking even though your all alone on the ship. I think the most spine chilling moment is when your on the elevator near the later part of the game. If you have your volume up high enough you can hear a woman singing Twinkle Twinkle Little Star. Scary as shit!

"There's going to be a monster around this corner... Now! Oh, there isn't... well then I'll just turn my back and-- AAAARGH! My throat! Why me?!"

Is basically what makes it scary. You make it calm, then startle, then make the person tense... slowly wait for them to calm again and then follow with some creepy messages to tense them slightly and force them to open the closet that contains the corpse. It does nothing, then when they move, the door shuts violently, the corpse comes alive... and it's old fashioned fisticuffs till it's dead. Or you're dead, whichever.

I just hate it when it rinses and repeats.

Gigyas from Earthbound. The scariest final boss ever. It's just a twisting, distorted red face that kind of looks like an ultrasound. The music is creepy and ambient, and you keep getting told that their is no hope and he is the embodiment of evil itself.

I actually find ANY part where you're getting chased by something that has the potential to kill you instantly pretty tense, as long as it's accompanied by suitable music.

When playing WOW, and the Warglaives when to a warrior instead of a rogue...
Or when playing COD 6 and I'm on a 24 kill streak just need one more for the nuke, but I'm out of ammo.
Or playing Pong where is shocks you if you lose that has to be the scariest moment in video game history. Real Pain > Fear

Eternal Darkness really spooked me on numerous occasions, but sometimes it was more about shock. Dear god, the bath scene...
Redeads and Wallmasters scare me to this day, though. The way they could jump out at any time, not to mention they're such irritating opponents.

if you look closer during the second part of the battle, you´ll notice that the "face" is actually a fetus.

Well, LeChuck couldn't actually kill you; he just zaps you to another room - doesn't make it any less scary though! :D

All of the Eternal Darkness atmosphere when you're completely batshit insane. People knocking on the walls, and doors, children crying, walls bleeding. That really set the mood for some scary shit.
