I've completed Ass Creed 2 to 100 and Spyro 1, 2 and 3, but that's about it.

my girlfriend COMPLETED Pokemon Heart Gold. I was dumbfounded.

Oh dear...so many to list...

-Metroid 1, 2, Super Metroid, Fusion, Prime 1 (working on Prime 2), Prime 3
-Innumerable Megaman titles (which aren't really hard to 100%...usually)
-Some stupid number of Mario games. It would take fucking weeks to recount them all.
-Duke Nukem 1 & 2 (that is, I found every single hidden cache of points conceivable. Some of those are deviously well-hidden)
-Baldur's Gate 2 (no, really. I've done it all. Took me a couple years) Actually proud of that one.
-I Wanna Be the Guy (well, you don't "beat" it per say; it's more like the sadist-dominatrix of video games)
-Gex 2 and 3 (couldn't find 1)
-Almost all of the game boy Castlevania titles (GBA and DS)
-Suikodens 1, 2, and 3 (only 100%'d 2 this year as I FINALLY found a copy of 1 to import data from)
-X-Wing Alliance (My legitimate ranking reached the PvP limit of Jedi Master; that's a lot of LAN play for a space flight game. Also, FUCK THE ESPIONAGE MISSIONS. Fuck those with a rusty spork! Holy shit are they impossible, and buggy as all hell!)
-Diablo 2 (no, really. I've succeeded in doing every conceivable build in the game, and then some. Both pre-1.10 and post. Pre-expansion too. Jeez...just saying that makes me feel older.)

EDIT: Yeah. I knew that wasn't the end of the list.

-Pokemon Blue (look ma! No PAR!)
-Goldeneye (N64)

There's probably many more that I cannot recall, but that's the main list.

I beat the original Ghost Recon on the hardest difficulty and then finished every challenge. On a 7" LCD screen. While in a war. Most of the challenges were easy enough, but the hardest required you to beat Red Square, on Recon, on the hardest difficulty, and all you could bring with you was the m24 kit.

To put that in perspective, you were forced to engage every single hostile in order to accomplish the objective. Your weapon required a full reload timer between each shot. There were ~50 enemies and you had 75 rounds to do the job, and unless you scored a head shot you weren't even guaranteed to kill a target in a single shot (even a round to center mass didn't always to do the trick!). To win you needed perfect accuracy, a perfect plan, and a fair amount of luck.

A list I am very proud of, some of those achievements took a long time.

The first Jak and Daxter game. Got 101%

And Kirby's Dream Land on gameboy. Beat it on normal and hard mode in one sitting.

I've caught Missingno before, and the only thing it did was screw up the graphics on the hall of fame, nothing else.

and on the subject of pokemon the closest I've come to 100% completion is on my Diamond version, all I had to get the highest trainer card colour was to beat the battle tower, but could never get past the 40 battle mark.

Also every mario kart game since 64, Super Mario 64 DS (never had the version on N64) Super mario bros 2 and 3, sonic 1,2, 3+K (thanks to sonic classic collection) Pokemon Collesieum (Including getting Ho-oh)
I probably have done more but I can't think of any others.

Havn't 100% any games in a long while, though I went through a long period of 100% games back on the PS2 though these stand out as being the best to me:

only ones i remember are Kingdom Hearts 358/2 days, Kingdom Hearts 2, and Super Mario 64.

I'm going for completing all the KH series now
Stuck on the clock tower boss on KH 1, stuck on maleficent on Chains, and i only just got BBS... Yea, this will take a while.

The only game I think i've ever managed to 100% was brutal legend, the style just really hooked me and I just had to do everything.

*edit* Actually I think I may have 100%'ed a few of the spyro games as well not, sure which though

Watch out for Brotherhood; each story mission utilises a synch criteria; completing the missions alone will only reward you with 50% synch. For 100% synch, you'll have to do things like not get spotted, or not to kill anyone but your target. For 100% synch it's incredibly tricky; I'm only on about 95%.

Harukanaru toki no naka de 3 and its expansions. For all its faults that game really drew me in. Definitely collected all the stills/events and endings!

As for sandbox games I'm pretty sure I've done all there is to do in Morrowind, Oblivion, and Fallout 3.

Splinter Cell Chaos Theory - every level completed to 100%, even the level where it's seemingly impossible not to get identified. And of course, Hitman Blood Money - silent assassin on every level plus acquiring, unlocking and buying every gadget and weapon.
