The second hardest would be Prinny, the hardest is literally a game called What Did I Do To Deserve This My Lord? 2. Both for the PSP oddly enough.

Gears of War, probably.
Last time I played I was working on Hardcore difficulty, and I kept getting stuck on certain places. Can't get past the gas pump station part... and it's not even the hardest setting!

I'm sure it has to do with my low patience however. Waiting for the perfect head shot and taking cover constantly isn't my thing.

Aye, I played on the easiest difficulty setting and I never managed to complete the game. Been almost two years since I played, but I believe I were close to the end. Gotta give it a new chance some day!

Well Rayman on the PS1 was more than impossiable for me back then but any recent game would be Demon's Souls world 5. The other 3 worlds are alot easier in comparrison however the fight with the Maiden at the end made it worth it. Lovely music and a fairly stright forward boss fight.

Baldur's Gate 1 and Final Fantasy Tactics the first time through. Once you know the metagame in both, they are much easier, but before that point, each game had my heart pounding through my chest in hopes that one slight factor wouldn't turn and allow my group to be ripped to shreds.

Trials 2: Second Edition. It's a combination of muscle memory and sheer determination. It took me hours of practice (i.e. failing over and over again) to become good enough to start not falling on only the medium difficulty tracks. Next, I fell over and over to become good enough to complete ANY of the hard tracks.

70+ hours of playtime in, and I'm certain I will never beat the last few hard levels. I've seen videos of people doing them... they're long and incredibly difficult-- and I can only get past the beginning parts once in every 100 attempts or I can't at all.

Trials 2: Second Edition, I will never beat this game; it's very near impossible for me. I swear those that have beaten it (without falling, no less!) either cheat, mod the physics of the game, are epic naturals at the game or have played the game for over 500 hours. At least, that's what it seems like while watching them own the hard levels' faces.

On the other hand, I haven't reached the later Dark Worlds in Super Meat Boy yet... I'm a bit afraid.

This. I've played many, many, many difficult games but this one takes the cake. I beat the game on it's first and second difficulties of four, but to this day cant get past the first bloody level on the 3rd difficulty (path of the mentor)

Demon's Souls was fairly easy, once you learned its cheapness and utilized Piccolo's training (DOOOOOOOOOOOOOOODDGE!!!!). Minus a few spots,it is quite easy once you master dodging.

Kung-Fu on the NES can be brutal, but Super Mario Bros 2 (known in the US as the Lost Levels) is as bad as a ROM hack. Mario can't make a good portion of the jumps (at least I couldn't make them as him), and Luigi slides a lot. Only reason I got anywhere was because of the infinite lives thing the Mario All-Starts version does for you. YOu lose all your lives, you get to start from the level you died on.
