The only thing of which I can think is in Morrowind, I set superjump to 99, axe skill to 99, but forgot to set agility (or whatever it is that makes you able to jump high WITHOUT taking damage) to 99 as well. I leapt in the air, aimed for Fargoth, and started humming "March of the Valkyries". I swung my axe, and at that moment, cleaved Fargoth, and hit the ground, killing me.

I still say it was awesome.

Also, Going through Sandopolis Zone in Sonic 3 and Knuckles without turning on the lights. That's the last time I'm ever going to take requests from my brother. >.>

I was forced to re-play like 50 missions.
it was still worth it.

My first playthrough of Fallout 3.

I was new to the "Explore world, Loot shit, Get XP" thing so I did pretty terrible my first time around.

I was helping 3-Dog by retrieving the Satallite in the History Museum. I killed a mutant and jacked his super sledge. I became over-encumbered. I had no idea what the words meant at the time so I assumed it was a unknown glitch.

I went through the entire Museum while walking at a steady .5 miles per hour.

Glorious.

The first time I played through FFVII I had just got out of Midgar and been chuffed to bits! (I had played other FF games and knew that this meant EPLORATION!!! :D) I was chuffed to bits and saved! It had felt like forever playing the first bit, and it didn;t help that I have played half the first bit before on a friends psx before.

I had another friend come over, and I showed him the game. We started up a new game, and he played the first hour or so while I watched and gave advice! I then saved it for him... I was so used to the order of buttons to press to save that I save. right. over. my. other. game!!!!

Left 4 Dead 2--In the carnival level, we come up on a Charger, and it picks me to...well, charge. At this point, I'm holding a katana, and although I could switch the my shotgun, my thoughts at the time were "kill it dead." So I charge at the Charger charging me, clicking my mouse as fast as possible. Stupid, yes...but not only did I cut down the Charger, I got an achievement for it.
Fable--"Well, now that I just got out of the Hero's Guild, I should go complete my first quest. Hey, look a guard...hmm. I have the weakest bow, but he's just wearing a cloth cap; that can't block an arrow. Alright, let's kill us a--oh crap."
Fallout New Vegas--The gun Thump-Thump looks remarkably similar to a rifle when you're paying attention to how the enemy's attacking you rather than how you're attacking the enemy. I wound up killing myself by launching roughly eight grenades at a giant radscorpion two feet away from me.

Demon's Souls.
After pressing the wrong button several times and failing at the same by falling of a ledge:
"OK. Remember. Do not press circle when you get there."
Walks up to edge, sees skeleton, presses circle, falls of edge again.

Also Halo 3 during SWAT:
(Me, to friend) "I'm going to shoot you in the head."
"No. You do that, I leave."
(Pretending to shoot) "Pew pew pew."
Finger slips and I shoot him in the head, instant kill. He left.

First thing that comes to mind is when I chased the Monkey King down and killed him, then instead of marching forward into the unoccupied upper lane, I double backed into the central lane and got ganked in HoN earlier today.

First console: N64
Third game: Ocarina of Time
First boss ever: Queen Gohma
Objective: attack the eye to kill her

I stun Queen Gohma, I run up to her face, and I slash her in the eye! ...only once. I wait, she gets up, and she starts attacking again. I repeat this process. She still doesn't die. This process goes on for hours, including several attempts as I get myself killed.

"WHY WON'T YOU DIE? I'M DOING WHAT YOU TOLD ME TO!"

Finally, I figure it out: I'm supposed to CONTINUALLY attack the eye. I have felt stupid about the incident ever since.

Infamous: Kicked a mail box for no particular reason, was killed on the fourth kick. Not by some baddy or something, just *plunk* dead, wake up on stretcher.

Playing Jedi Academy, opening the console and spawning a ton of Luke Skywalkers before changing my allegiance to the Dark Side... That experiment went really well...
