In Killzone 2, as much as I loved it, it always seemed like the meele was off. I haven't played anything multi-player that does that right, but in a single-player game, Zeno Clash is exactly this. There are a handful of ranged weapons that you use occasionally, but for the most part it's a first-person brawler, with various combos based on punches, kicks, finishing moves, and whatnot. Unlike almost every other first-person game ever, it actually feels right when you do those things, because of the way everything's weighted and the way the camera shifts as your character shifts their weight when lunging into a punch or kneeing someone in the face, instead of the usual half-assed melee stuff that's tacked onto almost every FPS. It's kind of short and very weird, but it's fun and sounds like what you're looking for. That's not always true. Look at Bad Company 2, where it takes a second to ready your knife so it isn't automatic. Look at Counter-Strike: Source, where two left clicks are the only way to get a knife kill on someone with full health and even then you have to equip it. Melee kills are good for being stealthy and for not wasting ammo. Just because some games have crap melee systems doesn't mean they shouldn't be in games at all. Think about melee in Left 4 Dead...
