Thief: Deadly Shadows-The Asylum level. Although I liked the ship more, the Asylum was way scarier!
Man... Shalebridge Cradle is one of the creepiest places I've ever seen.

Embarrassing to say but... In Oblivion; I was starting my character and was in the sewers. I thought I had cleared them of rats and since the brightness of my T.V. is naturally low a rat just hisses and jumps right at me.
Oblivion and Morrowind has made me jump more than once too. ^-^ You don't know exactly WHAT is going to be around any given corner. It's my second choice after Thief just because you are never sure if you are going to run into a rat, a trap, or a daedra at any given moment.

In MGS2, there was a trippy part when your commander kept calling you up and telling you to turn off the game, and that you were sitting too close to the T.V. & stuff. It sounds funny, but the way it was done, with the atmosphere & shit, it was scary.

In Fable 2, when I visited the little cottage in the snow
that was completely unexpected :D
I forgot about that! It was awesome. Very out of place with the jovial nature of the game.

The Giant Slaughterfish (a bit of a misnomer, more of a Large Slaughterfish. Still, though.) in the underwater cave for that thieves guild quest in Oblivion. I'd been playing the game for a good week or two, firmly believing the biggest thing in the water was a slaughterfish before this thing appears out of nowhere while I'm just trying to find a hole in the wall. Guess I was half right.

Keep in mind, I still have difficulties with the Sea Snakes in Majora's Mask. Oblivion's a lot prettier graphically.

And for the above post, one random house in skingrad is filled with undead, you go in expecting to steal silver plates and you find dread zombies and gloom wraiths.

The first time I encountered a ReDead in Zelda: Ocarina of Time. Damn, I almost shit my pants when I heard that scream, and the event that happened after that traumatised me for the rest of my life...

Possibly Viva Piñata and I'm not even joking. Smashing them to the ground with a shovel, while they lie there, dying and whimpering, until a crazed man comes up and smashes them to bits which in turn signals the coming of other piñata's, who devour the entrails of the crushed creature, regardless of it's relation to them.

Admittedly, I thought a bit too much into it, but it's still oddly creepy for that kind of game.

Wandering around the wastelands in Fallout 3. One time, I was walking to Tenpenny Tower when I hear a dog growling. I turned around, saw nothing, and as I turned back a dog freakin' leapt at me.

It's close to the one you buy. It's called "Nerastarel's house"

I'm going to go with the Spear of Destiny demo. On the first level, I think there was a glitch or something, but whatever it was, it would result in being able to hear doors opening and closing in the background, with the sound seemingly getting closer and closer.

I was young at the time, and I thought it was very scary.

As lame as this sounds, Disney games have some scary "game over" screens. And I'm talking about on the Sega Genesis. Aladdin comes to mind, with Jafar cackling with his big mug on the screen (the slot machine isn't my friend either). They also include this lovely screen before a couple of the levels.

And The Emperor's New Groove had a spooky game over screen as well. Children's games scare me, and yet I'm fine watching my brother play Resident Evil. Whatever. I'm not a fan of chasing games/levels or timed things, either.
