18+? Well, this is the US so 18+ is AO, and that would be Manhunt 2.
As for M (17+) ratings, it was probably Duke Nukem 64, but I only rented that back then...

It originally was but there was a popular mod released that made woman topless so they changed the rating to M even though you can't access the mod on Ps3 or 360

Marathon Infinity. I was only 4, maybe 5, when I first saw my dad playing it on his mac. I was already an avid SNES player then, having played Link to the Past and Megaman X till my eyes bled. I was aware that my dad had been playing a game on his computer and practically demanded that he let me see it. He did, and I was super freaked out by it. T_T (Though now it's one of my favorite games, so I suppose I owe him a favor for the mild trauma.)

I've never played an AO game, only Mature 17+ like Gears and Halo. I believe 18+ is only for pornographic games such as Lesiure Suit Larry and such.

It was either Fable of Jade Empire when I was about 11. Fable was first I'm pretty sure, but I got stuck after about 30 minutes. I remember playing Jade Empire and just being so shocked when those people were hit by cannon fire. Seeing so much actually blood in a video game blew my mind.

Now I remember playing Resident Evil 2 when I was about 6 on the playstation. It was terrifying.

I believe the first 18+ game I played was the original Halo, back in 2007 (Okay technically it was rated 17+, but still). Yeah, I know, but my parents did not let me play most any Mature game I wanted until I turned 17 so... Yeah.

Manhunt... that I know of anyway. Might have played some 18+ in the 90´s, can't remember what was classed as 18+ back then though.

Edit: Wow, looking at all these posts it seems playing 18+ games is all I've been doing my whole life.

Perfect Dark when I was, what, ten-ish? I was such a Lawful Good bastard in those days that when my brother's friend brought it over and invited me to try it, I protested the notion to a near-tantrum degree. It wasn't until my mom explicitly gave me permission that I cautiously gave it a shot... and promptly realized how much I was overreacting.
