My parents knew I could separate fantasy from reality so my first M-rated game was Resident Evil 2. I was 8 at the time.

Yes, that, I got it at release.
I wanted it because it got 10/10 in a game magazine I read at the time.
I do not regret that buy, God of War 1 is the best IMO. Sure, the others has got better graphics, so?

The first one I bought was Borderlands, just two months before I turned 17. I guess I just never really was too interested in any other M-rated games that my parents would've actually been willing to buy for me.

The first game I bought when I turned 17 was God of War Collection... entirely for the purpose of getting a game that I knew my parents would never let me play. Even though it's not as good as I had hoped, I don't regret my decision at all, because my mission was accomplished, dammit!

First quake when I was 7 or 8...

I played DOOM 95 alot earlier then that but like people are saying, dunno if that counts.

I think mine was Carmageddon and GTA 1 when they were first released. Fun times.

Resident Evil 3 and I don't remember the exact date but I think middle-school give or take.

if by first you mean played, then Mortal Kombat on either the Genesis or an Arcade cabinet before I turned 10. If owned is what you mean then it was Halo:CE for the original Xbox

Devil May Cry when I was 12. Thought I could handle the Curse Of The Bloody Puppets. I was wrong. They scared the fuck out of me. SO much that afterward, I put the controller on the floor and since it was a dualshock and there was no carpet, when I got slashed, the damn thing echoed in the entire room and scared me even more! Got over it the next day though.

Also, MK4. When I was 10. Jarek pulled a fatality themed-open-heart-surgery on me, scarred me once and then I spiked myself to death and got scarred twice. Needless to say, never played it for another 4 years and I have been hating Jarek the Jerk ever since.

American McGee's Alice and Clive Barker's Undying, in a special two-pack that Mom got me for Christmas when I was in... seventh? eighth? grade. My mom didn't care much then, but she seems to care a lot more now, when it comes to my stepsiblings.
