Looking back, this was kinda funny, but at the time it was freakin' painful. I was playing Mace Griffin: Bounty Hunter and I'd gotten through most of the game. I made my way through and came to a corridor which had a map change in it. Right before the map change, I literally fell through the floor. At which point it loaded the new map.
If you've never played Mace Griffin, it's a half-hearted port that doesn't let you quick-save, meaning you can only load your game from the last checkpoints. As I'd fallen through the floor when the map change occured, the checkpoint was now me starting the new map about 3 meters under the floor, where I continued to fall til I died. The only other checkpoint was to start the whole planet again, and I was NOT going back that far. In fact I uninstalled it and replayed Half-Life again. I've not touched the game since.
Like I say, I look back and smile now, because it's the only time a game has literally made it impossible for me to finish it, and to do so in such a bizarre manner is astoundingly poor on the part of the makers.

I have many many hilarious deaths, but I'll name only a few. First was Goldeneye 64. It was me versus the girl who lived next door, who is a truly epic Goldeneye player. One of her preferred weapons is the grenade launcher, and the kill that stood out was the one where she bounced the shell off the walls six different times before landing square on my head. To this day I have no clue how she managed that, but it's still hilarious.
Another is from Road Rash 64. Going down the road at 180 mph on a motorcycle, come around a corner and find myself nose to nose with a semi. Needless to say, the resulting head on collision was epic.
My last one is from Mercs 2. Trying to call in a rocket artillery strike via satellite, and I'm in the shanty town area, where all the buildings are the bloody same. After 20 seconds trying to find the right one, I say "Screw this", pick a building, and call in the strike.

A friend and I were testing out Mercs 2 multiplayer. I was in a Chopper, pretty high up, about 200 meters in the air, give or take. My friend was on the ground, and he was equipped with an Anti-Air missile. He knew I was at half health, but he thought bringing me down would be pretty funny. So, he targeted and shot me. In Mercs 2, when your health in a Copter reaches 1, you start spiraling towards the ground. I squeezed off as many machine gun rounds and missiles as I could before it was launch time. So I jumped out, and started my descent to the ground (realize that in Mercs 2, nothing can kill you in one blow; you get sent to 2 health and your screen turns red until you start to regenerate). I tried to shoot my friend, but Mercs 2 doesn't have friendly fire. I fell, remarkably, onto an enemy tank. I was hiding behind the barrel so no one would shoot me. Eventually, I emerged, still living, and the tank I was standing on was shot by an APC's TOW missile. It blew up and the physics engine kicked in, flinging me away like a ragdoll. Into fire. Burning me to death.

My friends and I were playing multiplayer Goldeneye, in one of the rooms ammo spawns in a toilet stall, and I was running low so I go to get some, but when I opened the door I died in a explosion because one of my friends had put remote mines all over the stall door waiting for someone to open it.

Myself and another guy, have just gotten into a helicopter, and are attempting to take off, when some pair of bright sparks in another heli (already in the air) decide to give us a scare: they nosedive into our just-taking-off copter, (apparently from nowhere) shearing it in half, and sending us flying into the water. We didn't actually die per-se, but I thought it warranted honorable mention, because of how hilarious it was. Friends and I laughed for a good 5 minutes after that.

Probably not my most hilarious but its the only one I can think I was playing halo 3 with about 3 friends and 1 of my friends kept on tryin to kick me out of my banshee he finally did and so i was thrown to the ground, I decided to follow him to see if I could get it back. Then he crashed into a cliff and jumped out and the banshee landed right on me! We both died!

GTA IV, I was on Xbox Live with a friend, and we had a 4 star wanted rating with 5-6 coppers on our tail whilst were on a PCJ. Suddenly a few crop up from the front so I bank hard right, and outta nowhere this cop car comes flying in. I lean forward on the bike just in time, however my friend who was gunning the cops behind us didnt realise this and gets splattered by the car.I start laughing so hard that I dont pay attention to the road n drive straight into a dead end and *SPLAT* dead.

Little game called 'N'. There are these user created levels, and there's one where you don't need to do anything, and you win.
I sometimes like to press a button to make him move, which usually makes his impale himself on a sharp edge, or just fly straight into a wall.
I remember laughing for like, 10 minutes over those deaths.

Well I have a funny one that needs some explaining first.
Me and a friend were pulling a all-nighter gaming session and had been playing Army of Two for an hour or so and the friend had noticed that the churchy bell sound effect that happens in it(If you've played it you know what I'm talking about) sounded like the Metalopacypse bell. So he says, "Hhhhhhhhhhhaaammmer!" like he was striking the bell anytime it happened.
Next we pop in Halo 3 for a little fun and he starts using the Gravity hammer, and the above quote. "Hhhhhhhhhaaaaaaammmer!" proceeded by my corpse flying through the air. Good laughs abound.
We throw in Unreal Tourny 3? I think and we were on the level with the bride, deathmatch.
This was the first time I've played the game an I was totally oblivious to sound effects that would make an experienced Unrealer alert for danger. I was walking down the lane on the bridge all big and bad cause I had a flak cannon. I hear a humming that is getting slowly louder. I ignore it. I turn around at the last minute to see a tank turn me into to roadkill.
Friend- "Hhhhhhhhaaammmeer!"
Me- "(speechless from my stupidity)..........wtf!?"

Uh... Split between oblivion and hitman: blood money.
In Oblivion I decided to dl a mod that adds a bunch of funny spells, such as changing people into animals, permanently, and dropping said animals on people, which consequencially don't do damage... Anyway... One of the spells happened to summon about 150 Angry Rufio's Ghosts, that just about attack anything. I cast it in the middle of the imperial city and watch as every inhabitant is shreded apart by these pissed off ghosts. Well, for some weird reason, the ghosts also could cast a weird form of a fire ball (this turned out to be a confliction issue with another mod) that does about 50000 damage and sends the character flying in thousands of pieces extremely fast. Well about 5 of those were cast on me. My character exploded sending thousands of little bitty elf pieces, litterally circling around Tamriel. I kid you not. I turned off the load screen (another mod) and watched my character fly around the world, which after a while either crashes or loops. I made it about 1 loop when the game crashed. I tried capturing it with fraps, but that crashed when my particles were sent on their flight.
My Hitman one is a little less buggy, but alot more innocent people were hurt in the process which earns a few more lols from me. So I decided to do the Mardi Gras mission (i forget the name but it has to do with crows) and i brought the assault rifle, tripped out to be a machine gun. So! Having a machine gun and being surrounded by people dresses in skippy outfits can only mean one thing... KILLING SPREE! So I walk around nonchalantly gunning down anyone who happened to be in my way. I did the math and apparently i killed about 500 people... Anyway when I ran out of ammo there were a couple police officers who wre shooting at me... So I'm like "f*** you." So I pull out my RC mine... Yeah... A horrible choice when fighting close range. So I toss it at the police officers who are in a gaggle of fightened barely clad women. Little did I know, but there was some sort of object with collision data that, yes you guessed it, caused the mine to bounce. Well... The good news was is I got the police officers, along with 30 other innocent people and me... Raising that days death toll from like 500 to about 534, 535 including me...

GTA4, playing against a bunch of guys in free play. They were going down a street in Algonquin, grouped together like a squad. I'm in the Annihalator and I'm coming at them down the street. I turn the chopper on its side so I'm coming at them sideways, right on the ground, and tip my blades into the street so I'm coming at them like a huge blade. It ends up working and I cut up most of them, then my blades finally brake, I go sliding down the street until I finally come to rest against a car. Little did I know the chopper was parked up against the front of the car unstably. So I get out of the chopper, shoot the one guy that survived my rotors, and then the chopper slides off the car onto me, killing me.
Funny story with those same guys, later they were all up on the roof of a building with AKs shooting up all my friends who were pinned down somewhere trying to return fire. I'm in a chopper (again) so I land on a nearby roof, they don't see me. I take out my sniper, and since they're all lined up on the roof I go down the line of 'em and shoot each one in the head.

i was playing GTA 4 and i was jacking someones car i didn't feel like running from the police so i was just pointing the gun at the drivers head so he would just get out but it was 1 of those guys who won't get out of the car so i went screw it and shot the guy then the car suddenly moves foward and kills me ( didn't have much health left )
I recently found a brand new way to die in TF2. I was playing as spy, and had just cloaked and skillfully dodged through a bunch of enemies. I ran down a narrow tunnel to hide, and hopefully get to the back of their base. There was an enemy teleporter in there; I had no choice but to run over it, but I was still cloaked, so I couldn't sap it. And, you guessed it, during the 0.04 seconds I was on top of it an enemy teleported in, and I died instantly. No idea who killed me, but he must have been pretty confused.
It was annoying, but also hilarious. I didn't even know TF2 had telefragging; it took me a few seconds to realize what had happened.

One moment in particular comes to mind. We saw each other at the opposite sides of the room. I immediately run and jump into the air, speeding towards him as he throws a single plasma grenade at me. I land on top of him and crush him to death with the hammer, and not one second later I explode. We looked back at the match later to see where he got me, his grenade landed on my freaking toe. My toe! We had a good laugh at it.
