This. Easily the hardest game I've ever played. I don't even remember making it past the third stage. And once you get to the end of the game, you have to play through it again in order to beat it.

I hear F-Zero games are really hard. Anyway, I'd say Advances Wars, normal campaign was really hard compared to most other games I've played. Probably all.

Over the top games? Probably I Wanna Be The Guy

Regular games? I found S.T.A.L.K.E.R: SoC to be surprisingly difficult, so much so that I failed the first mission several times.

Maybe I'm just bad at it.

Bioshock gave me a hard time, i'm sure if i went back and played it would be different but it sticks out in my mind as a hard game cause it freaked me out to play it when i first got it.

Mount & Blade (including Warband) set on "realistic" difficulty. For those who don't know, it will only save your progress on quitting - you can't reload and try again.

Heroes of Might & Magic 5. I never really did it right, but it's turn-based strategy and you tend to start off with one town as opposed to your opponents 3+. Given basic math, he'll simply outproduce you and become completely unstoppable within two or three weeks of gametime (which is about twenty turns), basically forcing you to rush and take out one or two cities within the first week of any given campaign map, usually with less forces than your opponent has. And this is on the easy difficulty.

Mind you, it's definitely possible, as people have completed the game on the heroic setting, but my logical mathematical mind can't cope with being at a constant and distinct advantage.
