For me, it was definitely Fable 2:

They make you sit through Lucien's motive rant and then, just as you're getting ready for him to pull out some last bit of magic, Reaver steals your kill. Goddamned Reaver. Steals Your Goddamned Kill. I never knew what a 'primal scream of rage' was until that moment.

When I was younger, we used to play Resident Evil Nemesis with a couple of friends. Now I know it might be funny now, but the mere sight of those zombies, especially the big fat ones, was the sickest thing I saw for a long time. I've become too desensitized towards violence, guts and the odd brain flying towards me nowadays, but yeah, at the time those zombies were the sickest thing.

yeah that was sick i agree but have you played l4d2? sometimes an intestine just flies through the air creating a big bloody line.
if you are squemish don't play it.
lol yeah I agree but usually I'm too busy bashing the zombies to examine the finer gorier details.

I would say either the L4D2 zombies with the pipe bomb, or FEAR 2 where if you shoot the gas canister on some guys' backs, it explodes and exposes their ribcage and spine and half their skull. Hilarious!

One of my favourite scenes from Bioshock, where you see the shadow of a Spider-Splicer playing with scissors and a corpse.
Then she disappears in a flurry of rose-petals.

Any combat in Fallout 3, as it usually ends up in the character being bombarded by brains, limbs, jaw bones, and bits of skull.

I walked in on my brother playing fallout 3 once.
You may or may not know what i'm talking about
But the first kinda town place has a drug addict woman, who from what i recall may be a prostitute. You can blackmail her by saying you'll talk to the bar owner in megaton.
And what my brother had done was killed her, took off her cloths, cut off her limbs and head and arrange them on a bed with other items.
I wasn't really freaked out at that moment and he was, but now that i think about it.

Well all the disturbing sprays I've seen in onlines games aside, probably the only two games that literally had me disgusted at certain points were dead rising and bioshock. The atmosphere in Bioshock was killer and let's just say that I absolutely hate scary clowns.

Funny you should mention that, I recently downloaded FakeFactory's Cinematic Mod http://www.cinematicmod.com/ and dusted off my old copy of Half Life2. Going through Ravenholm still makes me jump and the wails from the zombies when you set them on fire still makes the hairs on the back of my neck stand up! Nice (if a bit on the huge side; 12gb!!)) download that gives an old classic a bit more playtime! :)

Bunch of SICK deaths. Need an example? Oh, I don't know... a woman being strapped to the table, a torrent in her mouth, and her husband shoves (I think sheep?) intestines into it. When she starts choking on it, he just grabs a stick and shoves the stuff even deeper into the torrent.

I was thoroughly disturbed and disgusted. Divorces ruin lives, man.

Dark Seed comes to mind as a possible candidate. Never got further than the intro screen thanks to my DOS emulator bugging out on me, but that was enough to disturb me.

But yeah, there's the mirror scene in Silent Hill 3... I even guessed the mirror would try to creep the hell out of me, but it still managed to do it. I think I was playing it around 2:00 at night or something like that.
