Nobody~!

Well technically, that's not true. Legion, my tech specialist, died. Samara was fire team leader and because Miranda said it was a good choice, I thought it was the correct one. But because Legion was one of my favorites, I was all like, "AAAAHHH!! NOOOO! RESTART! RESTART! WALKTHROUGH TIME!" and discovered that Samara was the error. When I picked Garrus next time, Miranda was all huffy about it and I could tell it was because she knew we were screwing each other. Well shut up, Miranda! You misled me and got Legion killed!

Wait a moment, was Legion not loyal to you or something? Cause he or Tali are the ones you need to pick.

Nobody died in my first couple of playthroughs, but after that I've made a point of killing off Jack each time I play because I find her really annoying.

No one. I did every loyalty mission, and picked the choices I knew would work for the last mission. Garrus led the 2nd team, and tali was the technician. I also had a bit of foresight to see that the only reason for the ship upgrades was going to be for something at the end.

I played ME2 and couldn't finish it, mostly because ... the pink floaty alien things in the index aren't allowed to be kept as pets, an obvious bug...
...You're horrible! The Hanar are sapient beings, not animals!

Exactly, so they would make pretty cool pets. I was basically using Whatitsface as an attack dog, sicking him on enemies, mostly because he did it if I asked him, and partly because I suspected he had the mental capacity of a dog.

You see, it's a common misconception that the best ending is when everyone survives. I actually got the best ending, only Jacob died. The weird thing was I actually did his loyalty mission but I think I made a stupid decision about something and he ended up dead.

ummmm...
lets see, i sent mordin through the vent because i thought his being an amphibian would make him deal with the head buildup better, but his lack of good tech skills got him killed by that rocket at the door, then miranda wasn't loyal and she died at the end...i lost one other, but i dont remember.....
.....
might have been...Samara, i think i lost her, but i dont remember the circumstances...probably set as the squad leader around the seeker swarms.
yeah that sounds right.

Everyone lived (although it was tempting to send Jack into the vents...). Shepard don't leave noone behind.

Talia did when I sent her through the vents. I don't know why but whatever.

Garrus died when I took him with me on the "shield" part because the blue chick wasn't loyal (I failed her side mission, I can't even talk to women in a videogame :( )

Miranda died... Somehow. I left her behind with the getup when I got on the platforms.

I'm glad that guys like Legion and Grunt survived, and disappointed that Jack and the blue chick survived (you lost Garrus you *****) because I figured that I'd lose SOME people, it might as well be them.

I kinda wanted Thane to die because of the "plot cancer" thing so he'd have a heroic death, but I realized thatbmost of the deaths were "well, ______ is dead.... Moving on then". So it wouldn't work.

Nobody in either of my playthroughs. To be honest, I actually wanted some to die in my renegade playthrough just to see how ME3 handles the deaths of any squad members, but I don't know what you have to do exactly to get your squad killed.

Mordin, due to some stupid glitch with Mordin dying no matter what you do.
It has nothing to do with any "glitch".

At the end when you take two party members to face the final part of the game (including the human reaper), the squad members remaining back each have a "defender" rating unique to each character (with non-loyal squad members having a significantly lower rating).

If the combined defensive rating (measured in an average i believe) of the left-behind team is too low, then there is a chance that someone dies. This someone will always be the one with the lowest defensive rating, which of all the NPC's happens to be Mordin (if you did all loyalty missions that is, otherwise non-loyal squad members will have a lower rating).

There is actually a chance of him surviving if left behind, if the other characters left behind has a high defensive value (that would be characters like Zaeed, Garrus, Jacob etc. aka. strong fighters). You take some of the stronger ones with you leaving weaker behind, it might go wrong. The entire system is explained here: http://masseffect.wikia.com/wiki/Suicide_Mission#Suicide_Mission

The sure-fire way to make Mordin survive is to have him escort the Normandys crew back to the ship after you save them (or take him with you for the final part, which in most cases also means that the remaning squad members will have a high enough survival rating to make sure that noone dies).

@ Topic: Jack died on my loyalty Mission (on purpose). I never liked the *****, so according with the link above i posted, all you need to do to make sure she dies is to avoid purchasing the armor upgrade for the Normandy.

No one died on my first playthrough. I was a bit of a completionist when it came to loyalty missions and ship upgrades. However, Tali lived, even though I failed her loyalty mission.

The first time I played it Legion died after going through the pipes. For some reason I thought it would be a good idea to make Zaeed the leader of the first fire team. So I got pissed and reloaded a save from before I started the suicide mission and made Garrus the leader of both teams. That time nobody died... aside from my ship's crew, because I didn't know that if you dick around too long before the suicide mission they die.

Lost Legion as a technician because I chose Asaura as a squad leader. "figured she was a wise justicar"

Lost Zaeed when I sent him back with the crew " I did'nt know who else to send, he seemed like a decent choice"

Lost Miranda "lack of loyalty.

Delayed with crew and as a result lost half of them.

I loved the mission, everything felt like it had weight and I was always stressed that someone would die. That I'd made the wrong choice with one thing or another. I freaked out when Garrus "squad 2 leader" got shot and their was that half second of "is he gonna die? IS HE GONNA DIE?!?!?!" All and all a very satisfying ending. Loved it
