Abyssion from Tales of Symphonia. The final boss of ToS can have anywhere between about 35,000 and 50,000 health (I think, I know it changes based on your level). Abyssion has 120,000 and can use any of the main characters fighting styles, switching between them whenever he wants. He was really hard to beat but oh so worth it (I love Lloyd doing thousands of damage with one attack).

dear god was that hard.
Really? I never found him that hard. Just stay back and pick him off with your guns, then when he he attacks you just dodge to one move. If he does his tackle you jump to avoid it and then hack with your sword. Rinse and repeat.

There is a giant collector floaty reaper thing (not the boss) in ME2 that is fought in the derilect reaper ship level. That thing is super hard because husks come from everywhere forcing you out of cover while that thing rips you apart with its lasers.
If you're a soldier then I recommend using adrenalin and the black widow

This is the easiest question ever for me.
The Demi-Fiend in Shim Megami Tensei: Digital Devil Saga.
Even fully leveled, fully pimped, HE WILL KILL YOU.

The final Vergil battle in Devil May Cry 3. My fingers were numb and red raw by the time I got that sod down on DMD.

Dullahan from Golden Sun 2. I could only beat him once I reached level 60+ with my main players and 55ish with my off party.

Sephiroth from Kingdom Hearts 1. FUCK Sin Harvest. Again, had to be around lvl 60.

Ruby and/or Emerald Weapon from FF7. Still haven't killed them.

Isair and Madae at the end of Icewind Dale 2. They immediately gain all of the best buffs they can get, can't be dispelled, and have an entire roomfull of minions who make your life miserable.

Nightmare.
Gaxkang is a pussy. Wynne (or a PC carefully made with the right stats and spells) can one-shot him. And, he's especially a pussy compared to The Harvester on Nightmare, from the Golems of Amgarrak DLC. Still haven't beaten that mofo...

Dude, what? Seriously? He's a limp *****. Just use ZombieProof armor or cure the condition he gives you and slap him around with Physical attacks. Kill the Mortiorchis first and every time it comes back, it'll kill Seymour for you andd keep the Total Annihilation counter from triggering.

I'm not sure I can accept FFX bosses being in the thread as it's pretty easily possible to power-level the Sphere Grid and create at least one party of obscenely powerful strength and all the bosses have some variety of gimmik that's a key to victory.

OT: The hardest boss I've ever killed is hands-down, without a doubt

The Pain from MGS3 on euro extreme when you are out of grenades.

The Cyborg Ninja from MGS1......when you are also out of grenades.

i would have to say it is the final form of genesis in crisis core. you have to take on a deity the size of a 500 ft building while dodging his giant sword attacks and the spawns of creepy gremlins excreting from his sword. i could handle all that but what i couldnt handle was the games random style of lvling up. its basicly a slot machine where you have to match up certain characters/ summonings faces. you get 3 of the same face then the wheel takes up the entire screen and from there you need to have the number 7 match up 3 in a row. this is all random and no garentee that you will even level up. so i find myself constantly redoing levels in hopes of leveling up.
this boss is even harder than the secret boss who has 1,000,000 life points. you heard me, 1,000,000 life points. who honestly has time and pacients to actually see that fight out to the end?

As hard as ADOM and IWBTG are, the final bosses aren't really the worst part.

So instead, I'll nominate Yukari Yakumo, from Perfect Cherry Blossom. Let's say you beat the game on Normal with no continues - a feat in itself. Then you get the horrendously difficult Extra stage. Beat that too? Great! Now you get the Phantasm stage, which is the same but worse, culminating in the hardest boss of the entire series.

Keep watching, it gets worse with horrifying frequency.
This makes the dude who double-plays (one guy piloting two aircraft) Ikaruga look like a pansy. Also... Them patterns are beatiful, hypnotic AND DEADLY.

EDIT:

Yunalesca was tougher for me. I had a tough time aligning my turns so that when she transforms I can dezombify my party. For those who don't know, once she transforms, she will use a sort of Revive skill, which, if you're zombified, results in a total party kill. Also, thank god for making skippable cutscenes a standard these days. Auron's rousing speech was awesome the first time around, but loses punch after the nth time hearing it.

It's too bad that guy completely cheesed the last two phases. And he has the gall to upload such a shameful video! Pansy...

I was going to mention proud mode Sephiroth in Kingdom Hearts 1 and 2, but yeah, Yukari probably takes the cake.

Sephiroth only took me a week, maybe two. Think I was on Yukari for a good couple months before I finally beat her. Felt amazing when I did it.

Question: Do you mean hardest that I enjoyed or what TvTropes calls "That One Boss"?

Probably General Raam from Gears of War 1, or Seth in Street Fighter IV. Seriously, I couldn't beat him in Very Easy with Ryu >.<'

Those are the first that came to my mind, I may be forgetting one or two more.

EDIT: Ah!, I just remembered, Vamp from MGS2 in Hard. I couldn't beat the bastard.

Also almost every single boss in Devil May Cry 3, but Vergil at the very end takes the cake for the hardest boss in that game and that is saying a lot.
