Achievements that go against the gameplay.

By that I mean an achievement that tells you to do something that we not be beneficial in any way. This is very irritating if it's an achievement in multiplayer (an example is the medic achievement that involves ubering a scout).

Achievements that aren't skill involved.

Join a server with 6000 people? How does that prove that you accomplished something?

Get a 5,000,000 point combo in Tony Hawk's Project 8. (that's not a typo, it's a secret achievement. I know one may think I mean 500,000 points, but I mean ten times that many)

Completing levels and quests, you're supposed to do that, achievements should be extra.

Too many WoW achievements to name. Any of them that involve doing something with a certain number of friends.

To be honest I'm against achievements in general as they make the game less about having fun and doing what you want and more about pointless errands.

Says who? Though it is often better to use them as extra time wasters, but completing stories and quests are still achievements in their own right, and it is obvious you are referencing Fallout 3, and I actually enjoyed comparing what missions I did to my friends though now I have done em all so.. :p

Press the "A" button 2047 (I think) times in C&C3. Still haven't gotten it.

Most of the "Negative Achievements" in that game were pretty cool.

Ok, I see your point, main quests and levels are achievements in their own right, but I don't think you should get a achievement for just playing through the game, for me completing the game is enough of a reward, maybe one for that. Side quests are a good place for achievements because it's extra. as far as Fallout 3 goes, I'm a bit obsessed there, trying so hard to get every one, not easy.
