Shoot, beat me to it. Anyway, I have to say Enlightened Guitarist on Guitar Hero 3. Get five stars on every song on expert!? Thats impossible for many!
I can top that
Score 750,000 points on a song....0,o WTF? I have completed guitar hero on expert and done most extra hard songs and the most I've ever got is 503,000. Enlightened guitarist is possible through some easier songs but 750,000 is only achievable by nearly full comboing the devil went down to georgia or doing well on though the fire and flames.
I suppose they are real achievements though, they are ridiculously hard but the satisfaction for completing them is immense which is really what an achievement is about. Not just something you can get in 5 seconds.

Most useless one i've gotten to date was the one after the credits in Assassin's Creed.

when you walk around the empty "laboratory" and look at the walls and see strange markings, that are supposed to mean something, i never got why they were there though...

Those are awesome because you don't expect them. And it's Eat Lead, did you honestly expect anything but comedy and story based achievements? Just wait till you finish the game.

You aren't supposed to understand what they mean.

I hate any Achievement you CANNOT AVOID GETTING in a game, basically any achievement you get for completing a level.
It's like a patronising pat on the back for finishing that level, even though you HAVE to do that to get further.
A real achievement for me is getting five headshots in a row, collecting every token in the game or so.

On a related note, the Press Start achievement from the Simpson's game is the opposite, because it's NOT funny.

I'm tempted to say "partygoer" in Portal, because it's really just a part of the gameplay, but the little icon that you get when you complete it is just so funny...

Cadet rank 100 in Psychonauts was so freaking boring to get that I'm going to have to say that it's my worst. I must have done every level 100 times just to comb through every mind and get every single figment, psi card, and challenge marker. The extra cutscene for the effort was funny, but sheesh, it took me about fifty times the length of that cutscene in agonizing scavenger hunts just to get it. I'd suggest looking for the cutscene elsewhere if you care; they probably have it on youtube or something.

Completing levels and quests, you're supposed to do that, achievements should be extra.
Says who? Though it is often better to use them as extra time wasters, but completing stories and quests are still achievements in their own right, and it is obvious you are referencing Fallout 3, and I actually enjoyed comparing what missions I did to my friends though now I have done em all so.. :p
Ok, I see your point, main quests and levels are achievements in their own right, but I don't think you should get a achievement for just playing through the game, for me completing the game is enough of a reward, maybe one for that. Side quests are a good place for achievements because it's extra. as far as Fallout 3 goes, I'm a bit obsessed there, trying so hard to get every one, not easy.
its just to get people excited over wat they did
in Oblivion they keep track of info like monsters killed but don't useit for achievemanets which is a waste ofprogramming

That's what makes it an achievement.

So this cheesy little flash game seems relevant to the discussion of unnecessary achievements: http://kyon.pl/static/swf/10637.swf

If you aren't supposed to understand what they mean but they're rewarding you for seeing them, isn't that a bad choice in itself? If they're rewarding you for stuff you don't know just because you saw something about it, there's no point. It's like rewarding a kid for seeing a complicated equation. The kid doesn't know the equation, so what's the point of calling them a genius?
