Sacrificing yourself to save everyone was OK. What wasn't is how they made it so two companions could have done it for you safely but refused to as it was "your destiny". Simply removing them would have made it OK.
Normally I'm okay with the protagonist your playing to get killed off in a more linear type game but with a game like Fallout 3, you expect to be able to go back into the sandbox after the climax.
They should've at least warned you or just made an automatic save before you start that last quest.

Chronicles of the Sword. It was one of the first PS1 games out there.
Though it fit the game (in that it totally sucked) it still took ages to get there and it was just...ugh. To think they could have done so much with that game (and the ending!) and just let it fall through their fingers.

In more recent games though, I'd have to say...erm...Fable 1 and 2 (if only because after the "ending" there was absolutely nothing to do) and Mirror's Edge, which just kind of ends with a nothing.

No game should ever end with this phrase, let alone uttered by the main character, who should have a slightly better idea of what is going on than the player.

*Start of game* "So hey yeah there's this massive hidden vault full of alien tech and weapons and if you can find it you can basically be a god of gods with your overwhelmingly massive horde of weapons. You should get right on that!!"

*10 hours later* "So hey yeah you found the vault. Oh looks there's a giant monster inside it, you should go kill that."

*Kills boss* "Great, thanks for that... Oh here's the key to the vault. Go sell it to that woman over there for 2 million $ to add to the other 50 billion $ you already have and can't spend on anything."

....

"What?"

*END*

You just don't end such a hyped game with a cliffhanger. And the final boss wasn't all that great either.
At least Halo 2 had Halo 3 come out after it. KOTOR 2 on the other hand, well lets just say the ending was way worse then halo and they never released a 3rd game. That game makes me want to hurt the people who wrote it.

Borderlands. I thought you would at least get a lot of XP or guns or at least a lot of money but nothing. You fight an insanely easy boss and then it's just over.

Halo 2 is the obvious choice, but I'm going to have to go with the dark side ending in KOTOR 2. It made no god damn sense and left me with no closure whatsoever.

I'm surprised nobody's mentioned Bioshock 1, after an atmospheric experience, you fight some videogamey boss and it's the end. What!? I was expecting something deeper than that.

I heard that the development of kotor 2 was a mess and the publisher pressed them hard to release it before it was done so it would have the momentum of the original on its side, I think it was released like a year after the first
