repeatedly headshotting people on Black Ops. the conversation that followed the last time it happened (i was at a friends house and we were doing 2v2 with bots on one console)
Me: BOOM! HEADSHO- oh what the holy hell assfuck? (direct quote)
Friend 1 (Other Team): LOL ***** you suck donkey cock (again, direct quote)
Friend 2 (My Team): What the fuck happened, i saw the shot hit.
F3 (Other team): nah you just suck.
Me: oh fuck this sniper rifle *switch sniper class*.
F2: dont bother you're shit.
Me: *headshots* this one works fine...

More a collision detection problem.

Was playing the demo of Lara Croft and the Guardian of light (fun game). I was just faffing about a bit for fun. It was one of the maps you could throw spears on and stand on those. So i was horsing around with that near the wall when Lara suddenly fell through the wall and into the black abyss that was more or less non programmed space.

Not a story per say, but in general, my entire twelve hours on Crysis 2 multiplayer. My lord that was the worst hit detection I have ever witnessed.

Playing combat arms in the map called Junkyard, I ran up the middle of the map, up some stairs, shot a guy in the face, ran the length of a tunnel and then died, right at the top of the stairs that I had just run up. My lag was at around 5 seconds, cause I was playing on dialup.

I have plenty of stories in TF2. The worst being in Frontier, since the payload cart can carry people, and anyone standing on the moving cart is subject to some bullshit. I tried looking for a video, but let me tell you, anyone on the moving platform is almost invulnerable to backstabs and headshots, since the hit detection is so fucking shoddy due to the player moving like that.

Or the hundreds of time I drown an enemy in flames, and they walk of flame-fucking-free (while I look stupid, wacking them with the Axtinguisher). Meanwhile, enemy Pyros can catch me on fire from fifty feet away by so much as pointing their flamethrowers in my direction.

Oh, and let's not forget TF2's most famous example of hit detection -- the facestab. Stabbing someone in the back, through their face.

Or, since getting the Machina, I've learned that yes, TF2 does know I can get headshots (the tracer hits the enemy in the head, a blood splotch appears), but it does no damage. Same problem with using melee weapons: hit them, hear the sound of your weapons hitting them, do no damage.

Shit sucks, since it's mostly a lag problem, but goddamn, it's a lag problem that hardly ever goes away.

I could have sworn that my Blue Falcon completely missed Marth!
On a note you're more likely to understand, I've noticed that whenever I play DJ Hero online, my opponent is always a bit late on the crossfades, but still gets the points. Bizzarely, the taps and scratches are perfectly timed.
I'm not sure if that counts though.

It was taken from a youtube video, but I can't remember whose.
I also remember back in the days of playing splitscreen Halo on the original xbox, and to get a headshot with the sniper rifle all you had to do was draw the crosshair quickly over the enemies head, the autoaim would do the rest. It's good to see that in 10 years hit detection has improved so much. Errr...

Morrowind hardly counts, since the hit was registered by the system, but you obviously had a suck level of +20 (Just make sure to start with high agility with a weapon that you have actually focused on in creation. In the starting cave you'll miss say 1/3 hits at level 1.)

As for me, probably Skyrim. But with a sword. I don't care if my crosshair (That I made invisable) is slightly to the side bethesda, i'm swinging a sword bigger than an unkillable child. I kill everything in 2 or so hits anyway, so its no major problem, except when I try to stop a drauger from fussing my rother dahm big ass across the dungeon. And for some reason my character has polio so it takes him 10 hours to be able to stand up only to OH YOUR DRAGON SHOUT RECHARGED, GREAT. I swear if you rigged 6 deathlords up to a raft you could get to Akavir on Fus power.

In defense of my mudcrab misadventure I had been playing through the game quite successfully up until that point so I really think it was an evade bug or the like rather than hit shenanigans.

Well, sometimes you just roll a bunch of 1's because the game feels like being a dick. Thankfully "hit" is no longer left to spreadsheets in TES games anymore!

Though it was fun to drain Vivec of magicka and have him literally unable to punch you. As with most things Morrowind, they were annoying - But when it got around to exploiting them, it was pretty damn funny.

I actually did that really recently. Every time I have fought Vivec I have ended it swiftly and only recently realized that he actually has a finite magicka pool. Its fucking hilarious.

Speaking of exploiting the game to get hilariously pathetic adversaries, have you ever cast an aoe levitate (magnitude - 1) spell at a group of enemies? First time I did it I nearly cried laughing.
