The re-dead in Wind Waker, especialy because whenever you meet one they seem to have their own mini-movie dedicated to them.

the static on the little radio in silent hill. at the same time it was annoying but it made my stomach tighten like nothing else

That creepy fake talking created in Animal Crossing they made by just having the characters speak each letter in rapid succession. Occasionally the words will sound like the words they are, but some twisted notion of the word from some demon dimension, and other times it will just be a cacophony of chaotic noise burning your eardrums out from the inside... It makes some sort of disturbing cognitive dissonance where you know what you're hearing but it comes so fast that it sounds like something else altogether and you just sit down and cry for an hour until the UFO comes.

The sound the Dragon's Tooth in Deus Ex makes when it hits ANYTHING. The worst part is that it's the best melee weapon in the game, so you've got to settle for weaker weapons or GO INSANE. Or guns. Guns are cool too.

The witches crying in Left4Dead is okay but when she gets angry, before you actually disturb her, her grunting and growling is so annoying.

For me its got to be the chainsaw revving sound from Resident Evil 4. The bastard would pop up out of no where in a group of his buddies and gank you with one shot. Damn that was frustrating.

The crit sound in TF2 when there's nobody around, because it means you just got backstabbed.

Either 'You must gather your party before venturing forth', repeated over and over and over in Baldur's Gate 1, no matter how fast your characters are moving, or the sort of pre-explosion the Redeemer makes when it hits in UT1.

CoD WaW; the little "whumph" of a bouncing betty just before a hole is blasted into your torso. not only this, but going down the same corridor and hitting another 3 feet further down from the orginal.
