Looks I can be pretty liberal with, so long as she at least looks human I don't mind too much. Anyone who refuses to do something because it's too "mainstream" is immediately off the list, though, no matter how perfect she may be otherwise. Surprised nobody has mentioned this but the worst thing would probably be somebody who showcases any of the things I consider to be my own flaws.
The whole thing where you judge yourself more harshly than other people and somebody who says something and you go "wait a minute...that sounds like something i'd say and then get really annoyed at myself for saying".
That and people who talk only about their fave band/football team/horse 100% of the time, being passionate about something is cool especially if you're willing to put up with me being clueless but there really is a line.

I will not form a relationship with someone who is anti-religion. That particular bone of contention can split almost any couple apart.
Likewise, I would not want to date someone who does not respect me or my beliefs in general - disagreeing is fine, but the misguided conclusion that "anyone who doesn't agree with me is stupid/ignorant" is impossible to live with.
