Well its not exactly a game where you can die, but I was playing Lord of the Rings Battle for Middle earth and I on helms deep on the good campaign. The urukai tried to set a bomb at my wall but they put it back kinda far and my archers killed the berserker trying to detonate it. Trying to save myself from getting blown up in the future I made my archers with flaming arrows attack the bomb.....needless to say I blew the fuck out of my wall and about half my army thus screwing myself over.

I didnt work out how to beat the first riot guard in chronicles of riddick escape from butcher bay so i unloaded every bullet i had in the end i tried to punch him to death.

Awesome...GTA4, in a panic after being chased by a helicoptor in the bank robbery level, I failed to note the trajetory of the helicopter before I blew it up. Same outcome...

Playing CoD4, I threw a random grenade, which bounced of of some walls, and rolled right next to a car I was running past.

Fighting one of the tougher enemies in Fallout 3 I had ordered my companion to stay back (not sure why) and the enemies has a missile launcher. I run toward him and get hit a couple times and knock the weapon out of his hands. He pulls out another gun and I fight him up close. I finally kill him but I'm really low on health, I was about the pause it to use some stimpacks when I get shot and die! Turns out there was a Mr. Gutsy robot in the corner that I had failed to notice.

In Bug Too! for Sega Saturn, there was a part in which you had to launch yourself between Venus fly traps. I waited too long to launch to the next one (it goes back and forth fairly quickly) and I ended up getting eaten by the plant.

Anytime that I play like a JRPG or something along the line of that where you Forget to heal yourself before a freakin boss battle and they have taken me out in one turn...mainly my own fault but... FUCK YOU FINAL FANTASY! (I hold a grudge)

i made my first giant robot on garrys mod....it was huge! but the feet were weak and i left a g-man and a car on the level and i tripped and the exploding barrel ammo...did its stuff XD

Once in Zangband TK (Or whatever) I was running out of food, but I opened my pack to see that I had plenty. However, when I tried to eat the food it just "Fell through your jaws". It was just then had I realized the gods of Chaos were screwing with me, turning me into some sort of beast that couldn't swallow. So I made my way back up to the top level to go buy some scrolls of satisfy hunger.

Then it struck, my hunger had become so severe I had become paralyzed for a couple of turns until my strength came back.

This happened several times, but the scroll shop was a mere two squares away.

Then I became paralyzed.

Then my level 44 chaos beast fighter died (two squares from salvation) of hunger.

I've done a fiar bit of experimenting with spells in Oblivion. I'll let you figure that one out. On top of that, jumping where I should not jump always screws me over, expecially in games that run on the source engine.

One shameful death I experienced was in Left 4 Dead in the No Mercy Finale one Hard if memory serves. We camped beneath the stairs/runway that leads up to the helipad. We just about killed a tank but in the last few seconds of its death, someone threw a molotov that bounced off the tank and back onto us =/

I was playing Half Life 2:episode 1 and was in the mission where you first see the "Zombine".I used the gravity gun to throw the flares to see what the hell I was doing, where unknown by myself, there was a zombine with a grenade hiding behind a pillar (Damn subway levels) that I accidentally grabbed allowing it to replace my face with a cloud of molten shrapnel

Probably playing Wolfenstein about 10 minutes ago. I went running past a wall, and I died. Either the wall killed me, or one of the enemies messed up and glitched behind the wall.

Every time I play "The Mercenaries" mini game on RE4 (because I'm a trillion years old and still play RE4) and I go to the map with the double-chainsaw-dancing-giant I spawn right by him and I die within ten seconds. Ugh.
