Both of the following occurred in CoD4 multiplayer:
Trying to throw a grenade through a window, only to miss and have it bounce off the window frame and explode in my face.
Trying to go down a ladder, only to move backwards too quickly and fall to my death, three stories below.

On the long, bridge map with jumping lift things on each side, to send you to the other end of the bridge, I threw a plasma grenade whilst jumping into the lift, it hit a guy just coming off the lift opposite, and it exploded whilst we passed each other mid air, killing us both.

Pulling a "LEROYYYY A'JANKANS!" (ahem) on Halo and i turn into a piece of Spartan style Swiss Cheese. Also, my retarded brother not guarding the windows, cave entrance, and/or the door while I'm buying something from the "Mystery Box" in Call of Duty: World at War.
One of my worst deaths occurred while playing GTA-IV in cooperative multiplayer. We had gotten whats-his-face into the chopper. Two of our guys grabbed cars, while the other two (With me piloting) flew the chopper with the mob boss. We are almost at the landing pad (A baseball field) with plenty of time to spare. I am flying low. Apparently, slightly too low. The tires or the rotors clip the trees lining the field.
"Oh dear."
The end of the chopper disintegrates, leaving the chopper spinning in circles at high speed. Despite my efforts to slow down, the chopper is still going forward at quite a good clip. Anyone that has played this level knows what is past the baseball field. High way overpass. Fast forward: The chopper has now crashed on the overpass, and there are cops everywhere. We kill most of them in a desperate attempt to commandeer one of their vehicles. My compatriot goes down, but I manage to get the mob boss to one of the police cars.
Somewhat lost, I still manage to get oriented the right way, and am now on the island with the baseball field. I am driving a little recklessly due to the police chopper firing on me. This is where things went wrong.
Short of the field, the car hits the curb. Now, veterans of the GTA experience know this sometimes takes a little leeway with the physics engine. My car, having hit this 4 inch curb, leaps 30 feet into the air and flips over, smashing upside down into a tree at the edge of the field. As we are crawling out, the police chopper opens up. The car explodes. Mission failed.

Playing Grand theft auto San andress by jumping out of A helicopter as high as you can possibly get, jump out and then land in water. then having your helicopter hit you at A million miles per hour and die.
I died in CoD:WaW, a tank fired at where I was GOING to spawn, and the very second I spawn, it hits me.

Zobie Panic source I was pushing and then I died for no reason but it was kinda funny. Oh and another was TF2 as the spy. It was such a fail. I had about 5-6 people facing away from me and I was the spy. I tried about 4 stabs and I missed them all. I got munched bt a heavie's gun in 2 seconds after that.
The multiple times I have accidently thrown a greanade in either Rainbow Six Vegas 2 and Resistance: Fall of Man.
Spending ages getting to the top of the Shai Gen tower in Crackdown then jumping backwards (by accident) and watching myself fall for like 5 seconds of pure emotional agony

resident evil 1 remake. just took out a hunter was standing over its steaming body laughing...when another hunter jumps out of the camera blindspot and 1 hit kills me. i get to go back 2 hours and when i get there...i die again while trying to find the one hunter that killed me...>.>
Metal Gear Solid 2, the tanker chapter, the nerve-wrecking finale: Of all the ways to "game over" in that sequence, did I *have* to be so over-joyed at getting the last picture that I backed straight into the lazy, sleepy guard?
