Although there's a lot of internet argument about it, it tends to be agreed that the guy the Patrol caught, and the guy that stabbed Mellish are two different characters/actors. If you want more info, look for the arguments on "steamboat willie". But he is supposedly the guy that shot Captain Miller though.
As for the main thread:
Pretty much all the "good guy" deaths in Firebringer, that one always got to me as a kid, and still does to this day (especially Willow/Peppa (I can't remember which dies, but it's pretty heart-wrenching).
The death of Coltaine in Deadhouse Gates is pretty awesome too.
Yeah, most of my "good deaths" come from books. Can't think of many from films - those tend to be heart-wrenching generally (like the endings of Gladiator and We Were Soldiers) but no one specific moment really stands out.
