Garry's Mod really doesn't get enough attention. Months of entertainment that is constantly being updated by the community to expand the games past it's already incredibly huge level of freedom and diversity for a mere 10 dollars. In most games lets say you were confronted with a zombie. Your usual choices would be shoot it, beat it with a blunt object, or ignore it. In Garry's Mod you can: kill it, throw it across the map, put it in a race car, tie it to a rocket headed for the moon, befriend it, use it's body as a weapon to beat other zombies, build it a house, put it in a musical, or expand it to Godzilla size and build it a city full of NPC's for it to terrorize. The possibilities are only limited by the ingenuity of the community.

It should be obvious from by profile name, but in case you don't get it, I think Gish is one of the most unappreciated platformers ever, as well as one of the best indie games ever.

Indigo Prophecy - I know it has a cult following, but I still think overall it's underrated..
Def Jam: Icon - Maybe the mechanics aren't as good as Fight For NY, but they were decent, and the graphics and the turn table style added timing strategy to the game. I honestly don't see why people don't like it.
Silent Hill 4 - It wasn't the best, but people really don't give it enough credit.

Psi Ops: Mindgate Conspiracy (probably the most violent sci fi game I've ever played...and it was glorious)

Duel Masters (Closest thing to magic the gathering you can get without paying millions of dollars for cards or e-cards)

Metabots...don't remember which one.

Not the game itself, but the graphics: I consider Wind Waker to be the best looking game on Gamecube, and I'll willingly fight anyone who says different.

Earthworm Jim on the SNES (not the mega drive one, the controls on that one sucked!)
i lost HOURS of my life, on that game, and i've never seen anyone talk about it on teh intarwebs! it had some great humour!

Majoras Mask. Dear god why do people hate it when developers break out of the norm with their franchises. This game was great, story and game play wise, the whole 72 hours thing is NOT as tedious as people make it out to be. Hell this game was just as good as Ocarna of time, certainly more innovative. Guess fanboys don't like innovation.

saints row.most people just wrote off as a GTA clone.the only thing i hated about was the protagonist.(he would've been great if it hadn't been for if he hadn't been a near quiet retriever.)

Well, I'm sure everyone here appreciates Grim Fandango, but it certainly was under-appreciated by everyone else so that's my pick.

Me personally, my favorite underappreciated games are LoZ: The Wind Waker because I liked the art deco of it, decided not to go all gritty and dark and stuff, and Legend of Dragoon despite the fact that it got so much flak from critics and other people.

I think people need to understand better how awesome the Total War series is. You just don't hear about it much. I mean, not a single mention of Empire on G4's coverage of E3!
