While I played a handful of 18+ games when I was small(Duke Nukem, the early GTA games, etc) the first one that I owned myself was either Mortal Kombat Deception or Psi-Ops.

Isnt it 17 for all thouse games, or at least depending on place? Cause i've played a lot of M games ahem Oblivion after the nuts changed the rating, and not a single 18+ only game. Gotta keep my innocence you know.

I remember getting Max Payne when I was like 12 and thinking I was really adult, but think that one was a 15.
So my first 18... shit, I donno. Devil May Cry was a 15 as well, so maybe God Of War?
Ooh, actually I had Duke Nukem: Time To Kill when I was like 9 and GTA1 around that time as well, forgot about them

The first i played was probably Duke3D at the age of 15, as i only had an NES before that. However, I didn't like shooters back then, so i spent my time with RTS and adventure games until i bought Unreal Tournament (UT was rated 18+ and later put on the index in Germany). So i guess i'm among the late entrants to 18+ games.

StarCraft. It was actually rated M when it first came out...
Really? I'd swear i've bought it and it was a 12+ game here. Where do you live?

The first game I got when I became 18 was SR2, I didn't break no laws I swear!
OH, my, I remember Sega Rally 2 was that violent back in the day. Wait a sec...

Anyways, mine was Tenchu: Stealth Assassins for the PSX. One of the better 18 cert games. I did get Resident Evil a while before, but that was a 15 if I remember correctly.

Elder Scrolls 4: Oblivion. I had to practically beg my mom for it.
I'm sure it's a 15 but it might've been rated differently in America.

Back when I was like 4 or 5, I always booted up Leisure Suit Larry on my dad's 386 whenever I got the chance, just to get past the quiz designed to keep children out. At one point I had memorized every answer. I didn't ever get very far after actually getting in, though. I had no knowledge of English back then, aside from the test answers, so I got bored pretty fast. I suppose it was just the rush of doing something I wasn't supposed to do (although I don't think my dad ever cared because he knew I would get bored pretty fast with it).

Mortal Kombat Trilogy for the N64 when I was 7. I freaking liked it despite it being violent. It's kinda silly how my brother wanted me to turn off the blood and my mom didn't approve of him lending me the game but I wasn't affected negatively at all. Actually it was a good experience because I learned that mature games wouldn't affect me in any way.
