Its not really in the degrees of absolute terror, but in Bioshock when I was first playing the game, I went into the room that steams up in Medical Pavalion section. I grabbed the tonic and tried to walk backwards, finding that I couldnt, so I turned slowly, to see one of the splicer doctors just standing there looking at me.
I literally jumped off my seat at that, uttering "Oh Shit!!!" as I did so.
Maybe I just scare too easily...
Nah, I had that too! I really didn't expect it and so, I shit one. Also, later on in the same game with those creepy splicers that Sander Cohen has made to look like statues. I was hacking a safe and when I turned around, there was this splicer, standing still like a statue that wasn't there when I started! I panicked slightly...

The Tank music from Left 4 Dead makes me do that too, especially when I don't know where he is and he then suddenly appears really close to me. You'd think it'd be quite hard for a bastard that big to creep up on you...

Probably legend of zelda: ocarina of time with that big propellor thing in hyrule field. the first time i saw it i didn't know what to do so i was like... "noes, run."
I think dyanasty warriors 4 where you have to fight lubu first time is on par with it though. me and my cousin were just shouting, "what do we do god damn it!" Good times

Every time I saw Sephiroth in Final Fantasy 7, I was like 8 years old, and I hadn't quite grasped the intricacies of the cliches, patterns, and difficulty curves of games yet. Every time I saw him, I thought I was fucked (which sometimes I was, that game is HARD when you're 8 years old and don't know what the fuck "All" materia is)

The Dahaka in POP2. Of course you have to run, until the end when you get to show that guy who's boss!

My worst Oh Shit! Run moment happened in real life when I was a boy. I had just gotten home kicked off my shoes - literally - and one of them sailed through the air and gave my Mum a black eye. I was off and running and half way down the street before anyone caught up to me to say I wasn't in trouble it was an accident! Even then I didn't believe them. Frankly I thought I really was going to die!

There's a scene in the original Half-Life where you turn a corner in a tunnel just in time to see one of the huge colossus monsters flatten some guy against a wall with a truck.

Dunno about worst but i had 2 recently in Dragon Age: Origins. In both the Brood Mother and Archdemon fights i was the only person in the party and the amount of 'fuck fuck fuck' i had going on during that was ridiculous.
Also, that nasty abomination that wont fucking die in Dead Space always scares the shit out of me. Always

After the quest on Fallout 3 where you have to help the tree die after I did that I started to wander around that cave and out of nowhere I came across the really high level Murlurks (Sorry I know that's not the right name but I can't remember it right now) anyway I was able to kill them but soon after I ran into the king! Shit! I ended up shooting a Nuke at him then finishing him off with a Sniper Rifle bullet to the head after he did some damage to me. There was also the time in the Nuka Cola Factory when I cam across Nukalurks I was scared shitless lol, I exited a door and whenever I went through that door again they'd just be at that door so I had to kill them somehow.

My first ever moment like that would probably be Star Wars Jedi Knight: Jedi Academy, when you had to somehow eliminate the mutated rancor, you had to run from him while fending off hordes of cultists, it was quite intense. Also, the fight with Boba Fett from the same game, had to blow some ammo caches while he popped out once in a while to shoot you a new eyesocket.

And of course, every time you're not playing anti-tank on Bad Company 2 and a tank rounds the corner.

But I gotta be honest, there have been too many to pick out the worst one.

Basically every night in Fallout 3 with Fellout and that one mutant mod. So you're walking, scared to hell of everything, even that caravan that might just be a Super Mutant Gargantuan- You neve know, right?
So, you finally convince yourself that the caravan is nonhostile, then you catch something in your peripheral vision...Glowing...
You tentatively tap the 'v' button and...The screen is filled with enemy icons! Night Ghouls proceed to eat you.
*Shudders*
That sounds waaaaay too familiar to me...

Although my most recent Oh Shit Run moment would have to be when I realised I had just run out of ammo in Splinter Cell: Conviction....

Still getting my head around the new... well, everything. If I get a chance to beat Anna Grimsdottir to death with my fists, I will.

So I was in the Imperial City in Oblivion, and my health was a little lower than full. "Alright," thought I, "I'll just heal myself up." Forgetting to change the spell, I accidentally hit a guard with a fireball. Suddenly, I had every single guard in the city running after me, so naturally, I thought "Oh shit, RUN!" I didn't get very far.

Or the first time I came across a mummified Centaur in Tomb Raider 1. "Oh shit, RUN!" doesn't even cover it. I was literally gasping for breath while making Lara run as fast as was possible.

I am also going to say the Hunter from Dead Space. However, the 2nd time around I cheated and used infinite stasis.

Two come to mind - one quite late in System Shock 2, where a horde of zombies and spiders leaps out and chases you down a corridor (I can still hear that music); the other in RE4 when Saddler introduces you to "it" for the first time (in the suspended cage). That whole section was just one "Oh shit, RUN!" moment after another.
