Personally,I would make my family hate me before I died to spare them sadness,so my last words would be alone the lines of:
"I hate you all."

To be honest I think most people would just find that distressing. Telling people you hate them doesn't erase years of memory. No one like to be rejected.

I wouldn't be in a hospital bed. I don't want to die like that unless I'm asleep.
