I lost Thane the first time. I managed to fail his loyalty mission, somehow. Should have sent him back to the Normandy with the rescued crew, instead of Zaeed...

The the last level of Metal gear solid 4 is the best level ever

but other then that pretty much anything from Shadow of the Colossus

We don't go to Ravenholm - town of folks that got bombarded with headcrabs and the only survivor is a mad priest who sees himself as "the pastor" of all those "zombies"

Sandtraps - touch the sand and you die. (if that didn't make you curious, dunno what does)

Forgot to mention this one:

Mercury (Men Of War)

First level in the German Campaing. You start with 3 German soldier with pistols, nothing else. You have to kill and loot your enemies. I would have never believed that anyone would make this kinda level for a RTS game

What about the sniper mission in COD 4? Now THAT was intense on the first run.
'All ghillied up' was truly epic. I still remember the suspense when I first played through it. 'Death from above' was great as well. "Kaboom."

'Defeat Spirit of Motherwill' - Armored Core: For Answer (Taking that monster down just feels so good)
'Act 4' - Metal Gear Solid 4 (Nostalgia, Crying Wolf and an epic finale)
'Wolverines!' - COD MW2 (Running around suburbs with Predator Drones and Stingers was so much fun)

Tower of Latria (Demon's Souls). There has never been such relentlessly oppressive atmosphere in a videogame before. Ever. Now, maybe I should also mention the "sniper town" mission from Medal Of Honor, I know many people hate it but I loved it...rain, smoke, rubble and death at every step, it somehow came to life for me like real war might probably feel like. Oh yeah, also the Return to Kharak mission from Homeworld should probably be mentioned as the most emotionally intense moment in a strategy game ever.

Mission 4 (I forget its name) from Hitman 2: Silent Assassin. By this time the game has taken all the training wheels off, so if you get seen the level is over because they will execute the prisoner, but there is still plenty of options to complete the level steathly. Some really nail-biting moments as you have to walk calmly past the security cameras while not getting too close to any patroling guards.

Winter Contingency. I can't tell you how many times I've played and replayed that level.

If not that, then The Covenant. Huge mission that started epic and ended epic.

There's so many contenders... shalebridge cradle in Thief 3 was one of my first thoughts but I'm going to go with:

MGS 3 Snake eater, the fight against 'The End'

Just such a cool concept for a boss fight.

All the levels from The Club are well-designed and kick absolute ass, but If I had to pick one, then I'd say probably the warzone, as it is absolute CARNAGE.

A couple of levels from Gears of war 2 are really epic, such as the penultimate mission, where
You ride a Brumak, destroying everything in your path, and nothing can stop you whatsoever. It's time to take back your earth, and them locust know it!
and
That part where you fight the locust with the double edged chainsaw. That's quite cool

The Yavin 4 level of Jedi Knight II. It's just a very powerful Jedi you and a fuckton of stormtroopers. You can waste them left and right and still go down really fast, but if you're smart, it's just a massacre.
