Spoiler: You need to understand the context. Spoiler warning.

The lunatic is a former guard who got fired (in his mind unjustly) and as a revenge burned down his former workplace, a mall. He gets recruited by the Big Bad from the prison and gets hypnotized. Within his subcounsious, once you finally manage to free the Milkman from a group of homicidal girl scouts, he says the exact same thing: "I am the milkman. My milk is delicious." before starting to throw around Molotov Coctails disguised as milk bottles. The dialogue scene takes place right after you are ejected from the mind of the guard turned milkman by the carnage his subcounsious Milkman is doing to his own mind. The reason you had to do this to the guard is that he insists that he doesn't have the key to gate you need opened, he says 'I'm just a guard, the Milkman has the key.' Thefore, you go to his mind to find the bat-s**t crazy milkman and let it take control of his mind.

Lunatic: "I am the milkman. My milk is delicious."
Raz, looking at milk bottles the lunatic dug up from their hiding place: "Is that the normal kind? Or the exploding dream kind?"
Lunatic, with a twitching eye: "It is only fortified with what the world wants - what the world deserves.

Lunatic begins to walk away, carrying milk bottles and an insane grin on his face.
