AvP Classic 2000: In the Marine Campaign, when the aliens start to attack.. Freaking sneaky alieans.
Guitar Hero: When the screen starts to glow red.
Uncharted 2: Final boss fight.
Oblivion and Fallout 3/NV: In dungeons when things (mainly ghosts in Oblivion) start chasing me, I just run franticly while shooting/slashing.

I remember having loads of trouble with the last boss of Persona 2. It was a long fight so as the minutes passed I got more and more nervous. I didn't want to die and have to go through it again and again like I had. It was one of those moments in gaming where I was completely focused on the task at hand and nothing else mattered. Love those kind of moments.

I didn't know Final Fantasy existed until after I beat Kingdom Hearts 1-2, so I'd say the optional Sephiroth battle in KH2 at Hollow Bastion. I was level 73 and had beaten the game already when I saw this weird silver-haired dude looking over the cliff by himself. I had a couple potions/ethers and 1 elixir on me, and some munny so I figured he was selling something.

I walk over there, and he starts on about how nice my keyblade is? Then I got suspicious a bit, and was terrified after he confirmed my fears with these words.
"I wonder if it will change its mind once I defeat its wielder" [draws blade and One Winged Angel starts]

Me: huh wtf is going on "PRESS TRIANGLE"

I had the Oblivion key, fully leveled up Valor and Wisdom form, and good blocking sense so my style was to always hang back and read the enemy as clearly as I can to find opportune moments so it took 2 attempts to beat him. He killed me once because of that meteor storm thing caught me off guard. The first time he did that Zen Harvest thing that drops your health to 1 and magic to none was the first time my hands shook as I rushed to the elixir in my inventory. "Crap Crap Crap Crap!"

pretty much when ever I'm running from a big ass object and I dont know how close it is to me, and I'm just about to reach the exit. Happens a lot on tyostation on Gears. Just when I think I climbed on the platform. BAM! It got me, and I jumped.

Amnesia, dark Descent, eight hours straight of pure fear. Specially the pitch black unnaturally dark tunnels in Storage and the Prison, and the freaking bloody fog in the Chorus.

When Dark Falz first shows himself at the end of Phantasy Star Online. Seriously, I freaked out when he first appeared, and my heart kept racing throughout the entire fight. Not even De Rol Le (the game's second boss) freaked me out as much as Dark Falz.
