Tales of Vesperia.

Vesperia Master: Complete the game 100%

And it gives a whopping...

0 Gamerscore. WTF game.

That means getting every item, every skill, every cutscene, every arte and every single bloody skit.

That achievement from Duke Nukem Forever, where you have to meet the girl and have three kids.

I only managed to get that one using a walkthrough.

"With friends like these" in TF2. Basically, you gotta get 7 people in your friend's list to join a game with you. Yeah...good luck with that.
Stupidly easy on PC. =p

My vote for hardest, which I've actually done and took forever, is in FEAR. Achievement Real Time, beat the game without ever touching Slo-Mo. If you've played FEAR, you can understand how batshit crazy that was. Oh, and you're not allowed to play on Easy to unlock it.

Another which is hard as hell without achievement trading it is in TF2, Where Eagles Soar. You and another soldier or demoman both need to rocket jump and blow each other out of the air. BOTH of you need to die for it. So even if you rocket jump and tag a flying demo, it won't count. Most people get it by the Steam achievements bugging out, like it so often does.

I vote for the Platinum trophy in Demon's Souls.

That's the only achievement or trophy that sticks out in my mind as mind-shittingly difficult.

I wouldn't say hardest, but one of the ones that annoyed me the most is the "Seriously 2.0" achievement from Gears of War 2. Basically kill 100,000 enemies.
It's easier if you just go to the level where you hijack a brumak then fire away

The Frank the pimp and Saint achievements in dead rising were ridiculous hard to get.

You seems to have spelled Raining Blood incorrectly.

Aww, there there. =p It took me a while on PC too, but I almost exclusively play on a server where like 20 of the 32 players will be on my friends list.

Vidmaster Challenge: Déjà Vu
Complete Highway on 4-player Legendary LIVE co-op, with Iron, and no 'Hog or Scorpion.

Vidmaster Challenge: Endure
In Firefight, on any mission, pass the 4th Set on 4-player Heroic LIVE co-op.

These two are probably the two achievements I'm most proud of. Not only were they both easily two of the hardest achievements I've done but I got to do them with 3 friends and we had a blast drinking and talking all sorts of crap while doing them. Vidmaster Endure was insanely fun because it took hours and by the end of it we were all wasted haha

finish the challenge of the gods in god of war 1. it is pretty much 4 hours of yelling at ur tv because its based off more luck than any skill and it will make you want to pucnh a uppy because no matter what you do, no matter what you try, YOU WILL FALL off the edge or get pushed off or just die. needless to say, I felt the rage of kratos...

The achievement that has been taunting me for so long is "Heart Warmer" from L4D2.

It sounds so simple; leave the safe room to go back and and use the defibrillator on a dead team member to bring them back to life.

Too bad that situation never fucking arises. Either you get to the end of the match and everyone is still alive and kicking, you get to the end and you don't have a defib, or you get killed partway through and can't do it.

And then there is the one time where there is a dead team member close to the safe room, and you have a defib. So you make a dash for the room to qualify as having 'left' it, but as you cross the threshold the only other surviving member of the team shuts the door...and ends the round.

It's just one of those annoying achievements where the difficulty lies not in the task, but in hoping for the right conditions to arise where you can finally do it.

Also, on MW2, there is a title called "How the?" and to get it, you have to kill an enemy by shooting his own claymore through a wall.
If you run around with SitRep, it's much more likely. I earned it and think I've done it more than once actually.

Most tedious/time-taxing: Zombie Genocider from Dead Rising. But ~53,300 corpses later, you get THE REAL MEGA BLASTER!!! Psychos are less of a problem now.
Most tedious/time-taxing: The corresponding achievement in Prototype. In Dead Rising, you can just drive loops through the underground in cars and get kills pretty fast. In Prototype, there is no way to just rack up hundreds of kills in a minute reliably. And in Prototype, you don't get anything.
That Mega Blaster was sweet. I wonder if I still have my save files...
