For me it has to be completing Call of Duty: World at War on veteran difficulty, I spent so many hours effortlessly trying to complete those last 2 painful levels of grenade spamming AI and infinate waves of enemies. I eventually pulled through however, and was so relieved when it was finally done.
I still clench my asshole whenever thinking of how I got that achievement. Repressed memories...

I'm pretty sure it was 7 game days, which equated to many, many hours. Not only will you get bored killing zombies, the survivors (the best source of food) are all well armed and a complete pain in the butt to kill.

All your acheivments you guys are putting up HAS NOTHING!!! NOTHING!! on these two acheivments. There for armored core for answers. Get 10,000 kills, Get 10,000 wins. Just impossible unless all you did was play armored core.

Long before there were achievements, there were unlockable cheats. And I have never encountered one so rage-inducing as unlocking the Invincibility cheat on Goldeneye for the N64. To get it, one had to complete the Facility level in single player within 2:05 on 00 Agent difficulty (the hardest one). What made it so difficult? Well, one of the mission objectives was to meet up with a character named Dr. Doak. Dr. Doak could spawn at any one of five different spots in the Facility, and unless he spawned in the one spot closest to the main path through the Facility, it was literally impossible to reach him and then make it to the exit within that 2:05 timeframe, even at a dead run and ignoring the enemies. And given that the soldiers in the level are exceptionally tough and accurate at 00 Agent difficulty, even if Dr. Doak was in the right place at the right time, it was still a pain in the butt to make it through.

Oh, and you couldn't use any already-unlocked cheats to get any others. Lemme tell you, I threw the controller down and did a victory dance the day I FINALLY got through that door and saw a 2:03 mission-completion time on the debriefing.

Beating MGS4 on the hardest setting with no special items, no health items, no alerts, no kills, no nothing. That includes the section on the motorcycles which means you have to tranq enemies to death with a tranq pistol while speeding down a street on the back of a motorcycle.
Brutal.

Playing through Portal without getting hit even once. That is pretty hard. For me at least.

If you run around with SitRep, it's much more likely. I earned it and think I've done it more than once actually.

Aye, SitRep and the heartbeat sensor should get you that in no time.

The damn 'Velvet Rope' achievement in SR2. Levels four and above are impossible without another player!

If you found that hard to beat, you're doing it wrong. Walkthroughs apparently abound on the web, but I got it on my first playthrough without a guide. The point is to not fight zombies - you're supposed to simply hide in a corner, reading a book IRL, and chomping down on a steak every few hours.

The hardest achievement I've seen is quite possibly I Am Bad Company in Battlefield: Bad Company. As someone mentioned earlier, that requires some truly ridiculous feats. Like getting 5 artillery kills in one round, repeatedly, or getting 20 headshots in one round.

Honourable Mention: Soul Calibur 4 has an achievement to get a single Soul Crush kill, and an achievement for getting 100. I have yet to perform a single Soul Crush, because it's a stupid move that you only ever set up against a stupid opponent as a way of mockingly showing how much you own them. The AI's never set themselves up for it, so you need to play online to get it.

Ugh, I haven't been able to get this one yet; especially infuriating when the turrets say "gotcha!" after you've made it through most of the levels only to be pinged by one because you didn't get behind a wall quick enough.

Another Portal one is where you only go through the orange portals. I have trouble with this simply because I forget, and will turn around to double check an angle or something and poof! all your hard work is ruined.

Some of those 'ultimate weapon' tasks in Final Fantasy X are just ridiculous.

Dodging lightning? No problem.
200x? Oooookay.
In a row? Forget it.

woo! one of these i actually did! ive put way to much of my life into getting good at GH.

OT: beating ninja gaiden 2 on the hardest difficulty, and the series of like 6 achivements in that game requiring you to beat the entire game using only one weapon. there are like 6 weapons and 6 achivements for this.

oh god that is one of like 4 achivements i have left in that game. one i dont have the right guitar, 2 i dont have enough people to play with and the last one is this beast.

Midlife crisis, Tales of Symphonia; I DARE you to achieve that sodding title.

Also: 'He feeds on your tears' and 'Realm First - Anything': WoW, Pretty tough feats. There's other achievements that are just as tedious, but those ones require some real dedication and thought, and not just pure hammering head against a wall and a butt load of time on your hands.

It was 10,000, and i was at like 8.5k when my progress wiped itself :p

But my hardest achievement would probably finishing a level in Metal Slug 3 without miss.

Getting truly old school, but... finishing "The Last Ninja" on the C64, losing only a single life along the way (on the last boss, who was literally impossible to kill without dying at least once - his health bar stayed at whatever you'd beaten him down to).

The Godslayer, The Final Instance or The Last Remnant all from The Last Remnant. The require, in order, beating the ultimate optional boss (2 million hp I believe, `10 rounds or less. After round 10, he auto game overs you. He has attacks that hit the entire battlefield at once. Good luck.), beating every quest in the game (Quite a few are Guide Dang It's that you'll very easily miss.) and beating the ultimate form of the game's final boss. The Atlus developer's blog says they make their games hard because they get off on player's tears. Someone at Square-Enix took umbrage with that and decided to outdo them, because the TLR final boss is one of the hardest in his -normal- form, much less ultimate form.

Honorable mention goes to Armored Core: For Answer, which has an achievement that requires getting 10,000 wins online. Yeah, that's not a typo. 10,000. Online. Wins.
