The Earth, Wind and Fire achievement from WoW. I'll be very lucky if I ever get it done on my server (10 man or 25 man)...bah.

Here's where I look like a noob because I've only played 4 games that offered any kind of achievement. So here's a few of my hardest earned ones from WoW (of the other 3 games, 2 I haven't gotten any really uber achievements, the other was SWG and I can't remember what I did, lol)

Bloody Rare
Kill all of the extremely rare and hard to find Outland creatures listed below.

The Right Stuff
Learn the artisan riding skill.
(Note: I got this pre-2.4 when having an epic flier was a real accomplishment.)

Master Angler of Azeroth
Win the Booty Bay fishing contest or the Kalu'ak Fishing Derby.
(Done after the achievement was out, when everyone and their freaking cousin was doing the BB tournament)

Hand of A'dal
Earned the title, "Hand of A'dal".

And another:

"Banner of Brothers" for Team Fortress 2: you have to use the Buff Banner to buff 5 of your friends at once.

Yes, you have to get a certain amount of kills/assists as Soldier, use the Buff Banner, and have your friends who haven't died and are close to you stand under your Banner all at once.

Harder considering I have no Steam friends with the exception of one friend at school, who doesn't play Team Fortress 2 anymore.

That and "With Friends Like These" (play a round with 7 friends). I hate all achievements that require the use of a "friend".

From personal experience:

-The last level of World at War on Veteran (god-damn grenade spamming assholes)
-Mile high club on Veteran for CoD4 was an absolute piss-take; didn't stop me getting it, but annoying nonetheless
-Gears 2: 100,000 kills. This one mainly because I was 2/10 of the way there and the game completely reset itself. It thinks I have none of the achievements, when I do and starting that one again is just pointless.

I feel you. I went through those mines TWICE and came out with 299/300 grubs.

EDIT: Also, on MW2, there is a title called "How the?" and to get it, you have to kill an enemy by shooting his own claymore through a wall.

The Godfather: The Game- Achieving all execution styles. There's one where I have only seen literally one example of how to do it, no possible idea how.
The Ghostbusters Game- Haven't done either one, but beating the game on Professional difficulty and finishing with less than $100,000 in damages. The latter can be tweaked past, but the first I think would be impossible, considering the unnecessary/awkwardly placed difficulty spike in the last section of the game.

Pt in Prototype is annoying.

Oh, and I forget the name, but getting like 1,000,000 Proud Souls in DMC4 or something. That would take AGES.

Platinum trophy for Demon's Souls, let alone the collection and weapon leveling achievements. That game made me feel like a real gamer.

King of the Hill in Tiger Woods PGA TOUR 06. You have to 'Finish an online Tournament at the top of the leaderboard' which is even more impossible now that the servers are shut down...
Otherwise
XXL in Fight Night Round 4 the achievement requires you to 'Win the Heavyweight Championship Belt in Online World Championship Mode' There is only 1 belt in the world and you must defend your title every 72 hours in order to retain it or it is passed on, so assuming it passed on every 72 hours only 118 can get this achievement a year!

The feathers on AC2... I only have 7 more to get. After exams, they're mine!
MW2 on veteran - dear god that favela level. I just stopped halfway through it, as I was taking a good 15-30 minutes to get from checkpoint to checkpoint. It just throws enemies at you from strange angles... The number of times I've cleared an area just to have a new window open up with a hyper-accurate RPD wielding militia member on the other side...

And Seriously 2.0 on Gears 2... 100,000 kills? I'm only on about 9,200... I stopped then, happy in the fact that I was over 9000

Hardest attempted: Mile High Club from Modern Warfare - 20 some tries in, I decided it wasn't really worth it anymore - the controller was aimed at the TV, after all.

Hardest succeeded: At the moment, I can only think of Complete MW2 on Veteran, although I keep hearing that it was much easier this time around due to no infinite enemy respawns and less grenade deaths - though I'll tell you taht the firefight on the White House lawn was harrowing...

Most tedious/time-taxing: Zombie Genocider from Dead Rising. But ~53,300 corpses later, you get THE REAL MEGA BLASTER!!! Psychos are less of a problem now.
