So many. In Silent Hill 1 in the alternate school when the locker that's dripping blood is making noises as if something was strugling inside, and it turned that there was
nothing inside
Actually if you go and check that exact same locker when you are in the real world, a cat jumps out. XD

OT: My scariest moment is when... well hell, any level in Condemned. For some reason that game just scared the living hell out of me.

Condemned. The whole damn thing. Especailly the department store.

The ladder moment...F.E.A.R. 1
Seconded. Nothing like expecting to see "Something" at the top, and yet still get shock and awed by the scary devil girl.

The first time I played the Ravenholm level of Half-Life 2, I was almost too scared to even start it. There was this one bit in particular where I killed a bunch of zombies, turned into an alley to look for supplies, and a headcrab that I thought had also been killed jumped at me. Practically made me jump out of my skin.

Also, in Fallout 3, any time where I'm walking around the Wasteland at night is usually pretty unnerving, especially at the higher levels, where a deathclaw could very well jump at me from behind at any moment.
Why the hell did you go at night?

Don't forget the elevator moment.

In STALKER Shadow of Chernobyl I was in the area with army warehouses at night. I explore the countryside and find this abandoned town. My PDA told me there was a stash in there, so I went in cautiously. No hostiles on the radar, so I step into town. I get to the center and I heard a loud crash. I quickly pulled out my TRS 301, and loaded it. I check my radar, nothing. I get to the stash and hear a roar. I nearly shit myself. About 6 bloodsuckers (just ONE can kill you easily when its close) sprinted at me, fully cloaked, I only saw their blood red eyes moving at me in the darkness. I still don't know how I lived. Half the time I was spamming the use medkit button, the other half was running backwards and firing wildly at anything that moved... I don't like bloodsuckers.
Oh definitely, the Bloodsucker village would have to be one of my favourite scary-as-shit moments. I thought it was just creepy going into the tunnels under the Agroprom place because of the couple of Bloodsuckers down in there, but the Bloodsucker village really topped that one by several miles. Just hearing those roars from nowhere and then hearing them come rushing out of the abandoned houses, barreling straight through the boarded-up doors to get at me... Bastards.
It was funny for me in a way. I saw all the doors boarded up, heard a crash, and saw a hole in some boards. Luckily I had some other stalkers with me. However I can't go back their anymore. Even if I managed to kill all the blood suckers something strange is going on. I still haven't found what, but I have a rough idea.

Pretty much all of Siren: Blood Curse, until I realised the damn thing never changed. Zombies were always in the same place, weapons, ambushes...all the same on each play through. That first level with the cop...CHRIST! The escape route isn't even obvious, so I died several times, whilst crapping myself the whole time :D.

F.E.A.R, as many people have said...

OMG, does anyone remember Super Metroid for the Super Nintendo? Yeah, that last level with the Mother boss...freaked the hell out of me. I was only a kid! Getting the hell out of that space station with the lava rising all the time...

Thanks for the tips though, everyone. I'm gonna check out some of these games. Especially Eternal Darkness, now you guys have mentioned it!

Eternal Darkness is just the definition of "will spook you out."
PLAY THE GAME WITH CONSTANT LOW SANITY, AND YOU MAY JUST NEED A STACK OF CLEAN UNDERWEAR. Especially if it's your first run. At least now I know about that bath scene...

The whole marine campaign in Aliens vs Predator 2 was extremely scary, especially the bit where you're all alone in a series of corridors with nothing but a pistol on you and you're being stalked by a Predalien... then it bursts out of the ceiling.

A few games have spooked me out. HL2 and Ravenholm. Scared me shitless when I first played it years ago. I think it also affected me mentaly a little bit because when ever I play through again I always make sure I "set up a defensible baracade so the enemies[in this case zombies] won't get through" I now do this for every game I play.

Oblivion spooks me with every cave. I don't know what it is but they all scare me even though they are very similar. You would think I would get over it after the first few but no. Fallout 3's sewers never scared me like the caves even with the zombies, death traps and other nasties. I don't know what it is that make me affraid of Oblivion's caves.

Worst one would probably be FEAR. The first time I played it I left at the end of the first level because I was too scared and there isn't anything to do but sit back and watch in the first level. No actions or nothing. FEAR 2 didn't have the same effect on me. I completed that in a few days.

I was playing Eternal Darkness and there is a part in which you can examine a bathtub and your character goes temporarily insane and you can hear yourself scream and an image flashes onscreen of yourself laying in the tub which was full of blood. That freaked me out and it scared the crap out of my sister who was watching me play.
